Thickened, brownish, or yellowish discoloured nail plates that crumble easily suggest onychomycosis or dystrophic changes caused by chronic pressure.
Changes in nerve function manifest through specific patterns. Reduced vibration perception is often the first sign of distal symmetric polyneuropathy. Affected individuals notice that they can no longer adequately perceive the vibration of a tuning fork at the ankles or the first metatarsophalangeal joint. Loss of pain and temperature sensation follows later, drastically increasing the risk of unnoticed thermal and mechanical injuries.
Differentiating between various conditions requires clinical experience. A painful pressure point on the ball of the foot can be caused by a simple corn, known as a clavus. However, it may also stem from abnormal loading due to hallux valgus or metatarsalgia syndrome. Ischaemic pain resulting from peripheral arterial disease tends to occur during exertion or when elevating the legs, whereas neuropathic pain classically increases in intensity at rest and during the night.
Causes in everyday life
The causes of abnormal podiatric findings are varied and often result from the interplay between internal diseases and external influences. A central everyday factor is ill-fitting footwear. Shoes that are too tight, too short, or non-breathable generate continuous pressure and shear forces. This not only leads to reactive callus formation, but also promotes deformities such as claw toes or hammer toes.
Skin and nail changes are frequently encouraged by a warm, moist microclimate inside the shoe. Synthetic sock materials and inadequate ventilation provide an ideal breeding ground for dermatopathogenic fungi such as dermatophytes. Furthermore, incorrect nail care, such as rounding off the nail edges deep into the sulcus, regularly leads to ingrown nails accompanied by painful inflammatory reactions.
Systemic metabolic disorders represent the most serious internal cause. Permanently elevated blood glucose levels damage both the small blood vessels, termed microangiopathy, and the supplying nerve fibres via complex biochemical pathways. In rheumatic diseases, chronic inflammatory processes within the joints lead to subluxations of the interphalangeal and metatarsophalangeal joints, fundamentally altering pressure distribution across the sole of the foot.
Age-related tissue changes also leave noticeable effects. The plantar fat pad, which serves as a natural shock absorber beneath the metatarsal heads, atrophies over time. As a result, focal pressure on the tissue underneath bony structures increases significantly with every step. This favours painful callus formation and deep-seated hyperkeratosis.
When medical evaluation is necessary
A podiatric assessment serves risk stratification and refined follow-up monitoring. However, it in no way replaces medical diagnosis. As soon as signs of acute or progressive pathologies emerge during the assessment, an immediate referral to the treating general practitioner, diabetologist, dermatologist, or phlebologist is required.
The following red flags require prompt medical evaluation:
- Absence of pedal pulses in the dorsalis pedis artery or posterior tibial artery in combination with cold, pale skin.
- Open tissue defects, ulcerations, or deep fissures, particularly in individuals with diabetes mellitus.
- Signs of acute infection such as localized hyperthermia, spreading erythema, swelling, purulent exudate, or throbbing pain.
- Sudden blackening of tissue areas on the tips of the toes or around the nails, indicating tissue necrosis.
- Suspected Charcot foot, characterised by a warm, swollen, usually largely painless foot without preceding adequate trauma.
Interdisciplinary collaboration is the key to patient safety in these scenarios. Owing to regular examination intervals, the podiatrist often detects subtle deviations from baseline findings earlier than patients themselves. A written diagnostic report helps the physician interpret the findings and accelerates the initiation of necessary therapeutic measures.
What podiatric treatment can achieve
Conducting an initial assessment in the practice requires time, care, and specialized diagnostic instruments. Vascular examination begins with a bilateral comparison of skin temperature. This is followed by palpation of arterial pulses. The podiatrist palpates the dorsalis pedis artery on the dorsum of the foot and the posterior tibial artery behind the medial malleolus. To assess capillary refill, a capillary refill test is performed: following brief manual pressure on the pulp of the toe, the blanched area must return to its normal pink colour within two seconds. Delays indicate microvascular or macrovascular circulatory disturbances.
Three established diagnostic devices are used for neurological testing. The Semmes-Weinstein monofilament, a calibrated nylon filament, is applied perpendicularly to defined skin sites on the sole of the foot until it bends. This exerts a force of exactly 10 grams. If the patient cannot feel this touch with eyes closed at more than two testing sites, loss of protective sensation is present. The 128 Hz Rydel-Seiffer tuning fork is used to test vibration perception over bony prominences. Values below 5 out of 8 on the scale are considered abnormal. Thermal sensitivity is tested using a Tip-Therm device, which features a cold metal side and a warmer plastic side.

- Set up a well-lit area. If mobility is limited, use a hand mirror or place an unbreakable mirror on the floor to view the soles of the feet completely.
- Inspect the skin systematically from the ankle across the dorsum of the foot to the tips of the toes. Look out for redness, blisters, cracks, swelling, or dark discolouration.
- Gently spread the toes apart and check the interdigital spaces. Moist, white skin areas indicate a fungal infection or maceration.
- Pass the back of your hand over both feet to detect temperature differences. A significantly warmer foot may indicate inflammation, whereas an icy cold foot may suggest a circulatory impairment.
- Feel the inside of your shoes with your hand before putting them on. Foreign objects such as small stones, creased insoles, or protruding seams must be removed.
Caution is advised when selecting care products. Footbaths should last no longer than three to five minutes and must not exceed a water temperature of 37 degrees Celsius. Prolonged soaking severely softens the skin and damages its natural protective acid mantle. For moisturising, lotions containing 5 to 10 percent urea are suitable as they bind moisture in the skin. The interdigital spaces must be strictly avoided when applying cream to prevent creating a damp environment.
Textiles also play an important role. Socks should contain a high proportion of natural fibres such as cotton or merino wool and feature no constricting cuffs or thick seams in the toe area. Wearing white socks makes it easier to spot wound exudate or drops of blood that may have leaked from minor injuries without causing pain.

In clinical practice, the same typical care errors recur repeatedly, leading to severe complications. The use of sharp instruments at home tops the list. Blades, corn razors, callus planes, or nail scissors with pointed tips frequently cause micro-trauma when used improperly. In the presence of sensory impairment or poor circulation, minimal cuts can develop into chronic, non-healing wounds.
A widespread misconception involves the use of over-the-counter corn plasters. These products often contain highly concentrated salicylic acid designed to chemically dissolve hyperkeratotic tissue. However, the acid does not stop at the callus, but penetrates surrounding healthy tissue. In neuropathic feet, this regularly causes chemical burns, deep tissue necrosis, and secondary infections.
Self-assessment of temperature signals also carries risks. Many people with cold feet resort to hot water bottles, heating pads, or hot footbaths. When thermal perception is impaired, an impending burn is noticed only after blisters have already formed. Warming painful or cold feet should occur exclusively passively using warm, soft woollen garments.
Ignoring gradual symptoms is another mistake. Dry, scaling skin on the heels is often dismissed as a purely cosmetic issue. If deeper fissures, known as rhagades, develop from this dryness, the skin barrier is breached. Bacteria such as streptococci or staphylococci can enter, triggering erysipelas infection.
