What lies behind the condition

From an anatomical perspective, the human nail plate consists of around 100 to 150 layered strata of cornified keratinocytes. These corneocytes form a transparent protective layer over the highly vascularised nail bed, the nail matrix, and the hyponychium. A healthy nail appears pink due to the capillary network of the underlying connective tissue. If pigment deposits occur, cellular keratinisation is impaired, or transparency is lost due to cavities, the visual appearance of the nail plate changes. In medical terminology, a yellowish discoloration is referred to as xanthonychia.

The development of xanthonychia has diverse pathophysiological causes. In tinea unguium, classic fungal nail infection, dermatophytes such as Trichophyton rubrum or Trichophyton interdigitale penetrate the keratin layers via the hyponychium or the lateral nail fold. The fungi produce enzymatic keratinases that break down the structural proteins of the nail. The degradation products and metabolic by-products of the fungal colonies deposit within the tissue, fissure the structure, and lead to the typical cloudy, yellowish-brown discoloration.

A second common cause is discoloration caused by foreign pigments. If highly pigmented varnishes are applied directly to the nail plate without a protective base coat, organic colour particles migrate into the uppermost keratin layers. This pigmentation remains chemically bound to the keratin and only grows out slowly with normal nail growth of approximately one to two millimetres per month.

Furthermore, vascular factors play a role. In peripheral circulatory disorders, such as those occurring in peripheral arterial disease or diabetes mellitus, chronic under-supply of oxygen and nutrients to the nail matrix occurs. Consequently, keratinocytes form thickened, dysplastic keratin, resulting in clouding. The rare yellow nail syndrome also exhibits this effect, in which lymphoedema and respiratory diseases are accompanied by a reduced nail growth rate and thickened, curved, yellow nails.

Typical signs

The visual appearance of discoloured nails provides important clues regarding the cause. In dermatophyte infestation, discoloration usually begins at the free edge of the nail or at the lateral borders. Yellowish to whitish streaks form, spreading in a wedge shape towards the nail matrix. As the condition progresses, the subungual tissue crumbles, the nail plate partially detaches from the nail bed (onycholysis), and thickens in a crusty manner.

If, by contrast, discoloration is caused by cosmetics, the clinical picture is structured differently. Here, the yellowish hue appears homogeneously across the entire surface of the nail, usually on all toes treated with the polish. The newly regrowing nail area at the lunula shows a healthy pink structure. Thickening or softening of the material is completely absent in discoloration caused by nail polish.

Another condition is nail psoriasis, known as psoriasis unguium. This frequently manifests as so-called oil drop spots. These are sharply demarcated, reddish-yellow discolorations beneath the nail plate that resemble a drop of oil. These spots are often combined with pitted nails, which are small pinpoint depressions in the nail surface. Onychodystrophies following trauma, such as pressing footwear during hiking, can also appear yellowish-brown due to older haematomas as the blood pigment haemoglobin is gradually broken down.

A brief look at a typical case constellation illustrates the diagnostic process. A 58-year-old female patient presented to the practice because her left hallux nail had turned increasingly yellow over the preceding four months. She suspected a fungal infection following a visit to a sauna. Upon inspection, however, there was a localized discoloration without hyperkeratosis, accompanied by a clear sign of mechanical stress at the tip of the nail. This was a healing microtrauma caused by tight footwear, which healed without antifungal medication solely through pressure relief.

Everyday causes

In addition to pathological triggers, daily strains continuously act upon the toenails. Mechanical impacts from unsuitable footwear are foremost among these. If shoes with a toe box that is too narrow are worn, increased pressure is exerted on the nail plate and the underlying tissue with every step. This chronic compression damages the sensitive capillaries in the nail bed, leading to microhaemorrhages and keratinisation disorders of the matrix.

Foot deformities such as hallux valgus, digitus flexus, or splayfoot with fallen arches significantly alter the statics and rollover motion of the foot. The rollover forces are no longer distributed evenly across the forefoot, placing excessive strain on individual nails. Due to the persistent shear forces, the connection between the nail plate and the nail bed loosens, facilitating the penetration of moisture, bacteria, and fungal spores.

Care errors also play a role. Improper trimming of the nails, particularly cutting deep into the nail folds, damages the protective tissue. Pathogens thus find a direct portal of entry. Aggressively pushing back or removing the cuticle (eponychium) also opens the area above the matrix to infection.

Environmental factors and habits also leave their mark. Regular smoking, due to the nicotine and condensate contained, not only causes discoloration of the fingers, but also impairs oxygen supply to the nail matrix on the feet via vasoconstriction. Excessive use of acetone-based nail polish removers strips lipids and moisture from the keratin. The nail becomes brittle, cracked, and susceptible to the accumulation of foreign substances.

When medical evaluation is necessary

Although many discolorations can be managed podiatrically, clear medical indications exist that require specialist evaluation by a dermatologist or diabetologist. Warning signs necessitate prompt diagnosis to avoid irreversible damage or systemic complications.

A medical evaluation is strictly required in cases of:

  • Concomitant erythema, localised hyperthermia, swelling, or pain in the region of the nail fold, pointing to acute bacterial paronychia.
  • Dark brown to blackish-yellow streaks or spots that do not move forward with nail growth, in order to rule out malignant melanoma of the nail matrix.
  • Involvement of more than three nails or involvement of more than half of an individual nail plate, as systemic oral therapy is frequently required in such cases.
  • Presence of metabolic diseases such as diabetes mellitus or peripheral arterial disease, to prevent tissue necrosis and diabetic foot syndrome.
  • Immunosuppression due to medication or pre-existing conditions, where infections can rapidly spread into soft tissue.

Individuals with diabetes mellitus in particular should take any colour or structural change in their toenails seriously. Due to diabetic polyneuropathie, pain perception is often diminished, which is why inflammation under discoloured nails can remain unnoticed for a long time.

What podiatric treatment can achieve

A podiatry practice offers a broad spectrum of conservative and instrumental measures for restoring discoloured and damaged toenails. Before any measure is initiated, a thorough assessment is conducted. Here, the structure, thickness, and adherence of the nail plate as well as the condition of the surrounding skin areas are carefully documented. The first practical step consists of the painless mechanical removal of thickened and infected nail material. Using specialized diamond and carbide burs, the hypertrophic nail plate is debrided. This immediately reduces pressure on the nail bed and improves aeration. If the nail plate is extensively damaged due to debridement or trauma, nail prosthetics is utilised. Here, an artificial nail plate is modelled using medical-grade, flexible resins. Home care forms the foundation for permanently healthy and symptom-free toenails. Targeted routines can significantly reduce the risk of re-infection and mechanical discoloration.

Proper nail trimming technique is crucial. Always trim toenails straight across and only round the corners very slightly with a file. Cutting deep into the lateral folds must be avoided, as this damages the tissue and promotes ingrowing. Use high-quality professional nippers or emery boards for shortening instead of imprecise clippers.

When applying coloured nail polish, a high-quality, transparent base coat should always be used. This forms a physical barrier between the keratin and the colour pigments. Furthermore, grant the nails regular nail-polish-free phases lasting several weeks so that the keratin can dry out and regenerate.

Everyday hygiene protects against mycoses. Wash feet daily with a mild, lipid-replenishing soap and thoroughly dry the interdigital spaces in particular. Moisture between the toes favours the growth of dermatophytes. Socks should consist of materials containing natural fibres, such as cotton or merino wool, and be washed daily at a minimum of 60 degrees Celsius.

Shoe hygiene is also part of prophylaxis. Disinfect your shoes regularly with suitable shoe disinfectants and rotate footwear daily to allow worn shoes to dry out completely over a 24-hour period. Walking barefoot in public wet areas such as saunas, swimming pools, or hotel rooms should be avoided: use bathing shoes at all times instead.

Common errors

In clinical practice, we regularly encounter attempts at self-treatment that often aggravate the problem rather than resolving it. The most frequent error is concealing discoloured nails with persistent nail polish or UV gels. Underneath the air- and moisture-impermeable layer, a warm, moist microclimate develops that accelerates the growth of dermatophytes and Pseudomonas bacteria.

Another widespread misconception is the use of aggressive home remedies. Concentrated vinegar essence, citric acid, or tea tree oil are applied in high concentrations to the nail. This frequently leads to severe contact allergies, chemical irritation of the surrounding skin, and damage to the matrix, without the active ingredients reaching the deeper layers of the nail plate.

Excessive filing of the nail surface with coarse files also causes damage. Affected individuals often attempt to file away the yellow layer completely. In doing so, the protective layer of the nail is destroyed: the nail plate becomes paper-thin, sensitive to pain, and provides pathogens with even easier access.

Furthermore, the extent of fungal infections is frequently underestimated. Many individuals discontinue topical therapy as soon as the first visual improvement becomes apparent. However, as fungal spores survive in the deeper layers, a relapse occurs rapidly unless the damaged nail grows out completely.

Current studies and clinical context

Scientific literature demonstrates the complexity of nail discolorations and their multifactorial etiology. Studies show that onychomycosis accounts for approximately 50 percent of all nail changes [1]. Dermatophytes represent the main cause at over 80 percent, followed by yeasts and moulds. The prevalence increases significantly with advancing age, which is attributable to altered microcirculation and slowed nail growth.

Investigations into the efficacy of topical therapeutics clarify that monotherapy with antifungal lacquers alone achieves reliable cure rates only in mild infestations without matrix involvement [2]. As soon as more than 50 percent of the nail plate or the matrix is involved, clinical studies demonstrate the superiority of a combination therapy comprising atraumatic mechanical debridement and pharmacological treatment.

The role of microtrauma caused by footwear and altered foot statics has been thoroughly investigated in biomechanical studies [3]. It is shown that repeated shear and pressure forces lead to chronic onycholysis, which is almost always colonised secondarily by saprophytic germs and triggers discolorations.

More recent research emphasizes the importance of differential diagnosis. Not every xanthonychia is infectious. Systemic diseases, psoriasis, and adverse drug effects must be systematically differentiated to avoid improper treatment [4].

A reliable distinction between infectious onychomycosis and purely mechanical or dystrophische discoloration is not always possible on a visual basis alone. Targeted therapy therefore requires differentiated laboratory diagnostic clarification. In specialized practice, keratin material is collected from the affected area for this purpose. Using a sterile curette or scalpel, keratin scales are taken directly from the boundary between healthy and discoloured nail tissue, where the highest density of active pathogens is present.

The direct microscopic mount with potassium hydroxide solution, known as KOH preparation, serves as a rapid orientational aid. The microscope visualises fungal hyphae and spores in the softened keratin material within 15 to 30 minutes. However, this method does not yet allow an exact identification of the fungal species. For precise pathogen identification, a fungal culture is established on specialized culture media such as Sabouraud glucose agar. Dermatophytes such as Trichophyton rubrum require an incubation period of 21 to 28 days at a constant 26 to 28 degrees Celsius until characteristic colonies and pigmentations develop.

Modern molecular biological testing procedures increasingly complement these classic methods. Using multiplex polymerase chain reaction (PCR), the DNA of pathogens can be amplified directly from the nail sample. Within 24 to 48 hours, PCR delivers a precise result. It unequivocally identifies dermatophytes, yeasts such as Candida albicans, or non-dermatophyte moulds such as Aspergillus niger. This accurate species diagnostics prevents improper treatments with unsuitable antifungals and significantly shortens the path to effective therapy.

Systemic triggers and drug effects

Yellowish discolorations of the nail plate by no means arise exclusively from local factors on the foot. The toenail serves as a functional indicator of systemic metabolic processes and internal organ functions. When the matrix is supplied with altered metabolic products via the vascular system, this is reflected directly in the regrowing keratin.

Diseases of the liver and biliary tract can lead to yellowish pigmentation of all hard tissues via an elevation of serum bilirubin. Chronic renal failure typically presents with the picture of so-called Lindsay's nails. In this condition, the proximal half of the nail exhibits a whitish-yellowish discoloration, whereas the distal portion shows a reddish-brown hue. The cause lies in microvascular changes and toxin deposits in the nail bed. Endocrine disorders such as hypothyroidism also slow cellular renewal of the matrix, resulting in thickening and altered refraction of light in the nail plate.

Numerous pharmaceuticals affect nail growth and pigmentation. A well-known phenomenon is the intake of chemotherapeutic agents from the group of taxanes or anthracyclines, which lead to diffuse yellow and brown discolorations as well as onycholysis. Long-term administration of tetracyclines or synthetic retinoids can also trigger keratinisation disorders of the matrix.

A case example illustrates this connection: a 62-year-old male patient noticed a symmetrical, yellowish-grey clouding across all ten toenails following three months of medical therapy with a cytostatic agent. A mycological culture showed no pathogen growth. Following discontinuation of the medication, the discoloration grew out completely without trace within fourteen months.

Mechanical stresses in recreational sports

In recreational sports, particularly long-distance running and extended mountain hikes in the Allgäu Alps, extremely high shear forces act upon the forefoot. When walking downhill, the foot shifts forward against the shoe's toe cap with every step. These repetitive impacts compress the hyponychium and trigger microscopic haemorrhages in the nail bed.

The haemoglobin contained in the extravasations is gradually broken down in the tissue via biliverdin to bilirubin. This biochemical breakdown process leads to characteristic yellow, green, and brown tones under the firm nail plate. Not infrequently, this trauma-related xanthonychia is mistakenly interpreted as a fungal infection and treated unnecessarily with antifungals.

Moisture exacerbates this problem. Inside tight athletic shoes, a warm, moist environment develops that causes the stratum corneum of the skin and the nail keratin to swell. Macerated skin areas lose their natural protective barrier against mechanical friction. Frequently asked questions

Is every yellow toenail automatically nail fungus?

No, by no means. Although nail fungus is one of the most common causes, nail polish pigments, microtrauma from tight shoes, circulatory disorders, or conditions such as psoriasis can also trigger yellowish discolouration. Precise podiatric or dermatological diagnostics clarify the exact cause.

How long does it take for a discoloured toenail to grow back healthy?

Toenails grow very slowly, averaging about one to two millimetres per month. It usually takes between twelve and eighteen months for a discoloured nail plate to be completely replaced by healthy keratin from the matrix area to the tip of the nail.

Can I simply cover up yellow toenails with nail polish?

Concealing them is not recommended if the cause is unclear. Under a dense layer of polish, a warm and moist environment is created that promotes fungal infections and bacterial growth. Furthermore, polish makes it difficult to observe nail growth and can further weaken the keratin due to a lack of oxygen supply.

Do home remedies like vinegar or tea tree oil help against yellow nails?

Aggressive home remedies are not professionally recommended. They usually do not reach the deep keratin layers in sufficient concentration and can lead to severe skin irritation or contact allergies on the nail wall.
