What lies behind it
The venous vascular system of the lower extremities performs at maximum capacity every day. To transport oxygen-poor blood back to the heart against gravity, the body uses a sophisticated system of venous valves and muscular pumps. The veins of the legs are divided into a superficial and a deep venous system, which are connected to each other via so-called perforating veins. In an intact state, the venous valves act as check valves. They allow blood to flow in only one direction, namely from bottom to top and from the superficial into the deep system.
When the vein walls lose their elasticity or the vascular diameter widens, these valves no longer close completely. Medical professionals speak of venous valve insufficiency. Blood follows gravity and pools back into the lower sections of the legs. This altered blood flow considerably increases hydrostatic pressure within the vessels. The permanently increased pressure propagates into the smallest vascular beds, the capillaries. The delicate vessel walls are overstretched and lose their barrier function.
As a result, fluid, along with proteins and breakdown products, leaks from the vascular system into the surrounding connective tissue. Oedema develops. If this condition remains untreated for a prolonged period, chronic venous insufficiency, CVI for short, develops. The continuous tissue overload leads to remodelling processes in the skin and subcutaneous fatty tissue. Erythrocytes, or red blood cells, also leak into the tissue and break down there. The released iron permanently pigments the skin brown. At the same time, the oxygen and nutrient supply to the tissue deteriorates, which severely restricts the skin's ability to regenerate.
The muscle-vein pump plays a central role in venous return. With every step, the muscles of the calf and sole contract. They compress the deep veins and squeeze the blood toward the heart. If this mechanical stimulation is absent, for example due to prolonged sitting or foot deformities, the blood flow stagnates. The venous system is thus reliant on functional biomechanics of the entire foot.
Typical signs
Venous weakness develops insidiously. Initially, those affected usually notice only a feeling of heaviness or fatigue in the legs after long working days. The symptoms typically intensify in warm weather, as heat causes the blood vessels to dilate, making return flow even more difficult. As the condition progresses, visible fluid retention appears. These oedemas manifest primarily around the inner ankles and the dorsum of the foot. If pressure is applied to the swollen area with a thumb, an indentation remains visible for a few seconds.
As the disease progresses, the appearance of the skin changes. An early warning sign is corona phlebectatica paraplantaris, a fan of dilated, fine veins along the inner and outer margins of the foot. Later, brownish discolourations appear, referred to as purpura pigmentosa or haemosiderosis. The skin loses its elasticity, appearing dry, shiny, and scaly. In advanced stages, lipodermatosclerosis develops, which is a hardening of the skin and subcutaneous tissue. The tissue loses its natural cushioning function, making the foot susceptible to pressure spots and slow-healing wounds.
Differentiating this condition from other clinical presentations is crucial for correct management. Lymphoedema differs from pure venous oedema by the Stemmer sign. If the skin over the second or third toe can no longer be pinched or lifted, this indicates involvement of the lymphatic system. Lipoedema, by contrast, is a symmetrical fat distribution disorder that occurs primarily on the thighs and hips and spares the dorsum of the foot. In peripheral arterial disease, PAD for short, there is an insufficient arterial blood supply. Here, elevating the legs or applying compression increases pain, whereas venous symptoms usually subside with elevation.
The practice recently saw the case of a 58-year-old primary school teacher. She complained of increasing pressure pain on both balls of the foot and extreme skin dryness around the ankle area. Physical examination revealed clear haemosiderin deposits and incipient tissue induration. She had dismissed the changes for years as a sign of ageing and treated them with conventional cosmetic creams without addressing the underlying venous weakness.
Everyday causes
The severity of venous weakness is influenced by numerous everyday factors and anatomical conditions. Alongside a genetic connective tissue weakness, modern lifestyle habits represent primary causes. Lack of physical activity tops the list. Sitting at a desk for hours or standing on hard floors in retail inactivates the calf muscle pump. Blood pools in the legs, and the vessel walls remain under constant internal pressure.
Chosen footwear also exerts an influence on venous health. High-heeled shoes fix the ankle joint in a permanent equinus position. As a result, the calf musculature can no longer contract and relax fully during walking. The roll-over motion of the foot is shortened, which can reduce the efficiency of the muscle-vein pump by more than half. Footwear that is too tight or restrictive sock cuffs also restrict microcirculation and promote congestion.
Anatomical foot deformities permanently alter the gait pattern. Pronated pes planus (flatfoot) or transverse flatfoot (splayfoot) leads to altered biomechanics. The foot muscles tire more quickly, natural cushioning during heel strike is lost, and the interplay between the ankle pump and calf muscle becomes imbalanced. Venous return loses its mechanical support with every step.
Skincare errors aggravate the problem. Due to impaired microcirculation, skin affected by venous weakness is poorly supplied with moisture and lipids. If it is additionally washed with aggressive, alkaline soaps or dried with vigorous rubbing, the natural protective acid mantle breaks down. Cracks in the epidermis are the result. Such microtraumas serve as entry portals for pathogens and rapidly lead to chronic inflammation in the presence of venous congestion.
When medical evaluation is necessary
Venous weakness is not merely a cosmetic issue, but a progressive vascular disease. Certain symptoms require immediate phlebological or specialist investigation to avert complications. Sudden, unilateral swelling of the leg accompanied by pain, a sensation of tightness, or bluish discolouration of the skin is an emergency. Deep vein thrombosis is suspected here. If such a blood clot dislodges, it can lead to a life-threatening pulmonary embolism.
Similarly, localized heat, extensive redness of the skin, and painful induration require prompt medical assessment. It could involve superficial thrombophlebitis or erysipelas. Erysipelas is a bacterial skin infection that spreads rapidly when the skin barrier is compromised and may be accompanied by fever or chills.
Open sores, weeping wounds, or dark, necrotic skin areas in the ankle and lower leg region are clear signs of a venous leg ulcer (Ulcus cruris venosum). Such a leg ulcer will not heal without specialized medical and phlebological therapy. Evaluation by specialists in phlebology or dermatology is essential to accurately diagnose the cause and initiate stage-appropriate wound therapy.
What podiatric treatment can achieve
Medical podiatry plays an important protective and monitoring role in the care of patients with venous weakness. Due to reduced tissue perfusion and the tendency toward oedema, the skin on the feet and lower legs is extremely sensitive. Scissors, razor blades, or aggressive callus planes pose an unacceptable risk of injury in the presence of venous insufficiency. A small cut can take weeks to heal or, in the worst-case scenario, develop into a chronic wound.
In clinical practice, treatment within a podologische Komplexbehandlung (comprehensive podiatric treatment) is performed with high precision and in an atraumatic manner. Excessive callus formation, known as hyperkeratosis, as well as corns are gently removed using rotating diamond and stone burrs with continuous cooling or fine scalpel blades. Nail care also requires special caution. Reduced perfusion often leads to thickened, brittle, or deformed toenails. These are gradually thinned and trimmed in an anatomically correct manner without irritating the sensitive nail bed or nail fold.
If nail changes or ingrown nails are present, gentle corrective methods can be used to relieve pressure on the tissue. Continuous monitoring of the skin condition is also a key component of daily practice routine. Early stages of skin fissures, fungal infections, or stasis dermatitis are recognized immediately. Treatment intervals are individually adapted, usually ranging between four and six weeks.
A qualified podiatric assessment includes checking pulses, skin temperature, and the extent of oedema. What you can do yourself
The most effective pillar of self-management in venous insufficiency is consistent compression therapy. Medical compression stockings exert a defined external pressure on the tissue. This pressure is highest at the ankle and continuously decreases going upwards. As a result, the dilated veins are compressed, the venous valves close better again, and the velocity of blood flow increases significantly. The leakage of fluid into the tissue is effectively reduced.
Compression stockings must be put on in the morning immediately after rising, before the leg swells. When selecting them, care must be taken to ensure the correct compression class and fit, which requires measurement at a medical supply store. Flat-knit and round-knit stockings differ in weave and stiffness. For pronounced oedema or tissue induration, flat-knit garments are mostly used because they withstand tissue pressure better and do not cut into skin folds.
Alongside compression, regular activation of the muscle-vein pump is essential. Even simple daily exercises noticeably improve venous return. These exercises should be repeated several times a day, particularly during sedentary or prolonged standing activities.
- Foot rocking: Sit upright on a chair. Place your feet flat on the floor. Alternately raise your heels and toes as far as possible. Repeat this movement twenty times per side to activate the ankle pump.
- Toe gripping: Barefoot or in socks, try to pick up a paper tissue lying on the floor with your toes and hold it briefly. This strengthens the intrinsic foot muscles and supports the foot arch.
- Leg circles in a supine position: Lie flat on your back, raise one leg, and trace large, slow circles in the air with your foot. Change direction after ten rotations and then switch legs.
Skincare in venous weakness requires products specifically formulated for the compromised skin barrier. Conventional body lotions often contain too much water and too few lipids, which dries out the skin further after evaporation. Water-in-oil emulsions containing additives such as urea or glycerin in a concentration of five to ten percent are recommended. These ingredients bind moisture in the stratum corneum and improve skin elasticity. Cream should be applied in the evening after removing compression stockings, as lipid residues can damage the elastane fabric of the garments.
Kneipp hydrotherapy applications offer an excellent complement. Cold affusions over the lower legs reflexively constrict blood vessels. Blood is pushed toward the heart, and vascular tone increases. Guide the stream of water up the outside of the right foot to the back of the knee, pause briefly there, and guide the stream down the inside. Repeat the process on the left leg. The water should be comfortably cool, but not ice-cold.
Common mistakes
In clinical practice over the years, recurring behaviors have repeatedly been observed that inadvertently worsen venous conditions. Recognizing these mistakes is the first step toward successful symptom relief.
- Neglecting skin care under compression stockings: Many of those affected fear that applying cream will make putting on stockings more difficult. However, if the skin is not cared for at all, it dries out severely beneath the compressive fabric. Scaling, itching, and fine cracks develop.
- Exposure to high heat: Hot baths, sauna visits, or extensive sunbathing lead to marked vasodilation. The veins expand, venous return deteriorates significantly, and oedema increases substantially within a short time.
- Self-treatment of calluses with sharp tools: The use of blades, razors, or pumice stones on congested tissue frequently leads to micro-injuries. Due to poor tissue oxygenation, these small wounds often take months to heal.
- Incorrect nail cutting technique: Cutting toenails rounded deep into the corners quickly leads to ingrown nail borders in oedematous toes. The result is painful inflammation that spreads rapidly through the congested tissue.
- Wearing restrictive clothing: Trousers that are too tight, knee-high socks with hard elastic bands, or overly tight shoes mechanically restrict venous and lymphatic drainage behind the knees or at the ankles.
Current scientific evidence and context
The efficacy of medical compression therapy in chronic venous insufficiency is fully documented in scientific literature. Internationally recognized guidelines in phlebology classify compression as foundational treatment across all stages of the disease [1]. Studies show that adequate external pressure reduces vein diameter, improves venous valve function, and measurably increases blood flow velocity [2]. This leads to a rapid reduction in oedema and significant pain relief for patients.
Another focus of current research is microcirculation and epidermal barrier function in venous stasis. Studies demonstrate that chronic hypoxia in the tissue disrupts lipid synthesis in skin cells [3]. Therefore, the regular application of lipid-rich, urea-containing topical preparations should not be viewed as a mere wellness measure, but rather as medically necessary prophylaxis against stasis dermatitis and ulceration. The combination of compression, targeted exercise, and professional skincare forms the gold standard in conservative venous therapy [4].
Purely herbal preparations such as red vine leaf or horse chestnut extracts must be critically contextualized. Although some studies demonstrate mild oedema protection and subjective symptom relief, these preparations can by no means replace mechanical compression therapy in the presence of structural venous damage [5]. They can, at most, be considered as a supportive measure in early stages following medical consultation.

Once venous insufficiency has developed, it is generally not fully curable because overstretched venous walls and damaged venous valves do not regenerate spontaneously. How do support socks differ from medical compression stockings?
Support socks do not have a defined, medically controlled pressure gradient and are suitable only for individuals with healthy veins as a preventive measure during long journeys. Medical compression stockings, on the other hand, feature a precisely defined pressure gradient that is strongest at the ankle and decreases upwards. They are fitted according to exact measurements at a medical supply store and prescribed by a physician. Only medical compression stockings can therapeutically counteract venous oedema and tissue changes.
Why is the skin so extremely dry and itchy in venous insufficiency?
Increased pressure in the veins causes fluid to leak into the tissue, impairing microcirculation and the oxygen supply to skin cells. The skin can no longer be adequately supplied with nutrients and endogenous lipids, damaging the epidermal barrier. Moisture evaporates more quickly, making the skin brittle, scaly and itchy. Without specialised care, cracks easily develop, which encourage infections.
Which shoes are most suitable for venous insufficiency?
Flat, comfortable shoes with a flexible sole that allow the foot to roll fully from the heel to the toes are ideal. This natural rolling motion optimally activates the ankle and calf muscle pump. High heels should be avoided, as they fix the ankle joint and severely impede venous blood return. Furthermore, the shoe must not constrict the toes or the ankles.
How often should one visit a podiatrist for venous insufficiency?
For diagnosed venous insufficiency, a treatment interval of approximately four to six weeks is recommended. At this frequency, hyperkeratosis and nail changes can be safely debrided before painful pressure points form. In addition, we check the condition of the skin and tissue at every appointment to detect signs of complications or skin fissures early. Shorter intervals may be necessary in cases of acute skin problems.
