What lies behind it

A healthy toenail consists of several layers of transparent keratin, which are produced by the nail matrix beneath the proximal nail fold. During natural growth, these delicate keratin layers slide evenly forward over the well-vascularised nail bed. Under normal physiological conditions, the thickness of the nail plate on the hallux, the big toe, is approximately 0.5 to 1.0 millimetres. If the activity of the matrix cells changes or if an overproduction of horny substance occurs in the nail bed, the thickness of the nail plate increases significantly.

In medical terminology, this hypertrophic enlargement of the nail plate is referred to as onychauxis. In onychauxis, the nail grows uniformly thickened upwards without necessarily curving in shape. If the thickening is accompanied by a beak-like or horn-like deformity, podiatry refers to this as onychogryphosis. In this specific form, asymmetrical growth velocity within the matrix causes the nail to curve sideways or downwards like a claw.

Another clinical condition with a similar appearance is subungual hyperkeratosis. Here, excessive horny material is deposited directly beneath the nail plate on the nail bed. This often occurs as a defensive reaction to inflammatory processes or chronic fungal infections. A typical clinical case involves a 68-year-old female patient who observed a continuous increase in nail thickness following a sports injury five years earlier. The matrix had reacted to the trauma with a permanent keratinisation disorder, which, left untreated, led to thickening of over four millimetres.

Differentiation between these forms is crucial for subsequent therapy. While pure onychauxis often has a mechanical cause, thickening resulting from onychomycosis, a fungal nail infection, requires targeted concomitant antimycotic treatment. Without professional care, all forms lead to permanent alteration of the nail unit.

Typical signs

The most noticeable symptom of thickening is the loss of the nail plate's natural flexibility and transparency. The nail loses its delicate pink hue, which is created by the underlying, well-vascularised tissue. Instead, the keratin takes on a cloudy, yellowish, brown, or greyish discolouration. The surface often appears uneven, is traversed by longitudinal or transverse ridges, and loses its natural lustre.

As the condition progresses, the keratin mass increases to such an extent that conventional manicure instruments fail. When attempting to trim the nail, the material splinters or sharp edges develop, pressing into the lateral nail fold. Persistent pressure from footwear on the thickened plate gives rise to secondary complaints. Affected individuals report a dull, stabbing pain in the nail bed, which occurs particularly when walking downhill or after prolonged walks.

For accurate differential diagnosis, other dermatological conditions must be considered:

  • Onychomycosis: Frequently shows crumbly, friable keratin structures beneath the nail as well as white to yellow-brown streaks.
  • Nail psoriasis: Is often accompanied by oil-drop spots, pitting, or complete detachment of the nail plate (onycholysis).
  • Lichen planus: Leads to thinning or longitudinal ridging with scarring of the nail bed, rarely to an isolated increase in mass.
  • Peripheral arterial disease: Causes growth disturbances with fragile, extremely hardened, and slow-growing nail substance due to tissue hypoxia.

Careful examination of the nail and surrounding tissue allows the extent of layer thickness increase to be determined precisely and enables early detection of inflammatory changes.

Everyday causes

The triggers for altered growth patterns in the nail matrix are manifold. One of the most common causes is repeated microtrauma. Wearing footwear that is too tight, too flat, or too short in the toe box over many years subjects the tips of the toes to mechanical stress with every step. The nail is pressed against the toe cap of the shoe. This continuous stimulus causes the matrix tissue to produce increased amounts of keratin in order to protect the tissue against external pressure.

Acute trauma also plays a major role. An accident involving a falling object on the toe or a sports injury during football can lead to a subungual haematoma, a blood spot under the nail. If the sensitive nail matrix is permanently damaged by this pressure, it subsequently produces altered, thickened nail tissue.

Deformities of the feet and toes significantly alter static alignment and pressure distribution during the rollover phase of gait. Hallux valgus, in which the big toe leans outwards, or hammer and claw toes cause the nails to rub against areas in the shoe that are not anatomically designed for pressure loads. Biological age also influences the condition of the toenails. With advancing age, microcirculation in the capillary network of the skin slows down. The supply of nutrients to the matrix changes, which frequently leads to thickening and an altered keratin structure.

When medical assessment is required

A thickened toenail is by no means merely a cosmetic problem, but can indicate underlying conditions that require medical treatment. Prompt specialist evaluation by a dermatologist or orthopaedist is essential if signs of inflammation appear on the toe. Redness, localised heat, swelling, and throbbing pain in the area of the nail fold point to a bacterial infection, which can spread rapidly without therapy.

Particular caution is warranted if pus exudes from the nail sulcus or if dark brown to black discolourations appear that cannot be explained by a demarcated haematoma and do not grow out forward with the nail. In rare cases, a pigmented change beneath the nail plate must be histologically examined to exclude subungual melanoma.

Heightened vigilance is required for patients with diabetes mellitus, peripheral arterial disease, or neuropathies. In these individuals, even minor pressure points beneath a thickened nail can go unnoticed and lead to deep tissue damage, such as malum perforans (neuropathic ulceration) or pressure ulcers. Reduced pain perception prevents pressure points from being noticed in good time. Regular monitoring and treatment by qualified personnel are mandatory here.

What podiatric treatment can achieve

Medical foot care in podiatric practice aims to reduce the thickened nail to its physiological dimensions safely and painlessly. This immediately relieves pressure on the nail bed and effectively prevents secondary tissue damage.   • Diamond burrs: Used in medium and fine grit to smooth the nail surface after coarse reduction and to precisely round the nail edges.
  • Abrasive caps and ceramic burrs: Serve fine finishing work in the sensitive region of the nail fold and the correction of thinner keratin layers.

A typical treatment appointment lasts approximately 30 to 45 minutes, depending on the severity of the keratinisation disorder and the number of affected nails. The nail is thinned step by step from above until a smooth, supple plate remains that protects the toe without pressure. Should the nail be severely damaged by thickening or a preceding fungal infection, nail prosthetics can subsequently be considered to protect the nail bed from the lateral skin folds rolling inwards.

Treatment intervals are generally between four and six weeks. Within this period, the nail grows by approximately one to two millimetres. By keeping the nail plate flat on a continuous basis, the matrix is given the opportunity to produce healthier nail tissue under reduced pressure loads.

Home care significantly supports podiatric measures. A key element of self-care is softening hardened keratin using high-dose keratolytic substances. Preparations with a urea content of 20 to 40 percent are capable of cleaving hydrogen bonds in keratin and making the hard substance pliable.

Apply appropriate ointments or gels targeted directly to the thickened nail plate. Protect or spare the surrounding healthy skin using a light barrier cream. With regular application over several days, the uppermost horny layer can be gently filed down using a wooden nail file. It is advisable to avoid foot baths at extremely hot temperatures. Prolonged bathing causes the skin around the nail to swell significantly, increasing the risk of injury during subsequent care.

When selecting footwear, ensure adequate space in the toe region. A thumb's width of space should always remain between the longest toe and the inner tip of the shoe. Avoid shoes made from stiff, unyielding upper materials or with overly narrow fits. Socks made from natural fibres such as cotton or merino wool reduce moisture accumulation on the foot and prevent a microclimate that favours the growth of fungal spores.

Common mistakes

In clinical practice, we regularly encounter attempts at self-treatment that worsen the problem or lead to painful injuries. The use of unsuitable tools tops the list. Household scissors, wire cutters from the hardware store, or dull nail clippers crush the thickened nail plate instead of cutting it cleanly. This creates longitudinal cracks extending deep into the sensitive nail bed, forming painful entry portals for bacteria.

A widespread error is cutting the corners of the nails too deep. Affected individuals often attempt to cut deeply into the lateral sulcus of the thickened nail to relieve pressure. This almost inevitably leads to remaining nail spicules embedding deep into the soft tissue as they regrow, triggering a highly painful inflammation (unguis incarnatus or ingrown toenail).

Equally critical is excessively aggressive scraping of the lateral nail fold with sharp metal instruments. This damages the delicate eponychium, the cuticle at the nail margin, opening the door to pathogens. Finally, purely cosmetic coverage with thick layers of nail varnish without prior reduction traps moisture beneath the colour layer, providing ideal growth conditions for dermatophytes.

Current evidence and medical context

Medical research confirms the necessity of regular mechanical reduction of thickened nail plates to maintain foot health and prevent secondary complications. Studies on onychomycosis treatment consistently show that monotherapy with topical antimycotics in severely thickened nails achieves low cure rates, as the active ingredients can barely penetrate the hardened keratin layer [1]. Only the combination of podiatric debridement followed by topical therapy leads to significantly higher cure rates.

Studies in older patient cohorts also demonstrate a direct link between hypertrophic nail changes, gait instability, and an increased risk of falls [2]. Due to altered pressure pain, affected individuals unconsciously adopt a compensatory posture that negatively affects the rollover motion of the foot and reduces walking stability. To relieve the painful hallux during the push-off phase, affected individuals unconsciously adopt an antalgic posture and shift their body weight to the lateral border of the foot. This compensatory supinatory movement alters the rollover kinematics in the subtalar and ankle joints. The abnormal loading propagates up the kinetic chain and can trigger postural misalignment in the knee, hip, and lumbar spine regions.

Orthopaedic aids and pressure-relieving measures

Mechanical reduction of the nail alone is often insufficient when static foot deformities continuously maintain the causative pressure. In such cases, custom orthopaedic devices supplement primary podiatric care. Custom-moulded silicone orthoses are used for direct pressure relief in the shoe. These are sculpted directly on the patient's foot using medical-grade silicone putty with a Shore hardness between 20 and 35 Shore A. A precisely fitted toe-cap orthosis prevents the thickened nail from impacting the shoe leather with every step.

Orthonyxic nail brace treatment provides another form of care. If the nail is not only thickened but also rolls laterally into the nail sulcus due to the altered keratin structure, corrective braces made of spring-hard stainless steel wire with a thickness of 0.4 millimetres are applied. The instrument is precisely adjusted and anchored to the individual nail shape. Through continuous spring tension, the brace gently elevates the lateral nail margins. Pressure on the nail bed is relieved, and the nail matrix is given the opportunity to regrow in a flatter form.

Long-term prophylaxis and aftercare

Following successful reduction of nail mass, permanent stabilization of the nail unit requires a structured aftercare concept. A central element is transitioning home care from high-concentration active ingredients to long-term hydration. While urea concentrations of up to 40 percent serve keratolysis in the acute phase, emulsions with a urea content of 5 to 10 percent are sufficient for daily prophylaxis. These keep the regrowing keratin supple and prevent the nail substance from drying out again.

Strict measurement criteria should be applied when purchasing footwear. Foot measurement is ideally performed in the late afternoon, as foot volume increases by 3 to 5 percent during the course of the day due to gravity and fluid retention. The interior of the shoe must exceed the true length of the foot by an allowance of 12 to 15 millimetres to prevent the tips of the toes from bumping during the swing phase of gait. Upper materials should feature high water vapour permeability so that relative humidity inside the shoe does not permanently exceed 80 percent.

For vulnerable groups such as older adults, athletes, or individuals with metabolic disorders, a follow-up interval of six to eight weeks in a specialist practice is recommended. At these intervals, minor keratinisation disorders of the nail matrix can be ground flat at an early stage before significant substrate build-up recurs. Continuous inspection ensures that altered pressure patterns are identified and corrected in good time.

Frequently asked questions

Can a thickened toenail become thin again on its own?

No, spontaneous healing generally does not occur with thickened toenails. Once the nail matrix has been damaged by trauma, chronic pressure, or ageing processes, it permanently produces altered keratin. Targeted podiatric reduction is required to restore the nail plate to a normal thickness. However, if the underlying causes are treated early, the growing nail can develop in a healthier manner.

Is filing down a thickened nail at the podiatrist painful?

Does vinegar or another home remedy help against thickened nails?

Furthermore, the acid can irritate the surrounding skin and lead to inflammation of the nail fold. Medically tested high-concentration urea ointments are significantly more effective and gentler on the tissue.
