What lies behind it

Swollen feet during pregnancy are primarily the result of a finely tuned interplay between hormonal changes, altered pressure conditions in the vascular system, and a significantly increased blood volume. From the first trimester onwards, the blood volume of the expectant mother increases by up to 40 to 50 percent. This increase mainly affects blood plasma, which alters blood flow properties and increases fluid pressure in the capillaries. At the same time, the hormone progesterone causes dilation and relaxation of the vein walls. The veins lose tone, which impairs the return transport of blood from the lower extremities to the heart.

In addition, the steadily growing uterus in the pelvic cavity exerts direct mechanical pressure on the inferior vena cava. This backpressure extends into the small veins and capillaries of the legs and feet. Due to the increased hydrostatic pressure, liquid blood components leak into the surrounding connective tissue. Medical professionals refer to this as interstitial oedema. These fluid accumulations follow gravity and are therefore particularly pronounced on the dorsum of the foot, the ankles, and the lower legs.

In addition to pure oedema formation, a biochemical restructuring of the musculoskeletal system takes place. The hormone relaxin is secreted in elevated concentrations. Its main biological task is to soften the ligaments and tendons in the pelvic area to render the birth canal flexible. However, relaxin does not act locally alone, but systemically throughout the entire body. Accordingly, the ligamentous support structures of the foot also become more flexible and yielding.

The human foot skeleton possesses two main arches: the longitudinal arch on the inner side and the transverse arch in the forefoot area. These arches are stabilised by a complex network of ligaments, such as the long plantar ligament (ligamentum plantare longum), the plantar fascia, and supporting muscles. If these ligamentous structures loosen under the influence of relaxin, combined with continuously increasing body weight, the arch of the foot gives way. The foot flattens and spreads in both length and width. A collapse of the longitudinal arch leads to a functional flat foot, while the yielding of the transverse arch promotes a splayfoot.

Typical signs

The most characteristic symptom of fluid accumulation is pitting oedema. If light pressure is applied with the thumb for a few seconds to the swollen dorsum of the foot or the shin bone, a visible dent remains, which refills only slowly. Affected individuals usually report a feeling of heaviness and tension in both feet. The skin appears smooth, shiny, and is more sensitive to friction. Socks leave distinct indentations, and shoes that fitted without problems at the start of pregnancy press painfully against the toes or instep.

Due to the mechanical collapse of the foot arches, the entire gait pattern changes. The contact area increases, which often requires shoes to be chosen one to two sizes larger. Overloading the overstretched plantar fascia frequently manifests as sharp heel pain, which occurs particularly during the first steps in the morning. Pressure also shifts to the forefoot, promoting the development of calluses under the metatarsal heads.

It is essential to distinguish these physiological adaptations from pathological conditions. Symmetrical swelling of both feet, which partially regresses after elevating them overnight or following mild cooling measures, is considered typical. However, if swelling occurs suddenly, massively, or exclusively in a single leg, the finding requires medical investigation. Unilateral swelling accompanied by calf or popliteal pain, warmth, and redness of the skin may indicate deep vein thrombosis. If oedema increases abruptly and also spreads to the hands, face, and eye area, medical diagnostics must be carried out immediately.

Everyday causes

In daily life, various factors act as accelerants for swollen feet during pregnancy. Prolonged, uninterrupted standing or sitting is paramount. In these positions, the calf muscle-vein pump remains inactive. When contracted, the calf musculature compresses the deep veins and essentially pumps blood towards the heart. If this pumping mechanism fails, blood rapidly accumulates at the lowest points of the body due to gravity.

Inappropriate footwear significantly exacerbates the problem. Tight, rigid shoes impede the natural rolling movement of the foot, inbalancing the calf muscle pump. Flat ballerina flats or flexible canvas shoes without a supportive footbed provide no support to the capsule-ligamentous apparatus softened by relaxin. The foot arch collapses unhindered under the load. On the other hand, high heels lead to shortened calf muscles and extreme overloading of the forefoot.

Thermal influences also play a role. Warm summer conditions or excessively hot baths lead to peripheral vasodilation, the widening of superficial blood vessels. As a result, even more fluid pools in the tissue. An underestimated factor in daily life is altered posture. Due to the forward shift in weight, lumbar lordosis increases. This change in alignment tilts the pelvis and alters the angle of the legs, negatively affecting pressure distribution on the sole of the foot and venous return flow.

Finally, limited reach to the feet in the final trimester leads to care errors. When trimming nails becomes anatomically difficult, toenails are often rounded off diagonally or cut too deeply at the sides. Due to swelling of the toe segment, the remaining nail edge quickly presses into the softened nail fold. A painful inflammation ensues, an unguis incarnatus (ingrown toenail), which can become chronic without professional care.

When medical assessment is required

Pregnant women should carefully monitor changes to their legs and feet. While moderate, uniform oedema is usually harmless, certain warning signs require immediate consultation with a specialist in gynaecology or emergency medicine.

A sudden, extreme increase in fluid retention within a few hours or days is the key symptom of pre-eclampsia. This condition, formerly known as pregnancy gestosis, represents a serious complication. Medical evaluation is imperative upon the appearance of the following symptoms:

  • Sudden increase in oedema on the feet, legs, hands, and face
  • Blood pressure values persistently above 140/90 mmHg
  • Severe, persistent headaches or visual disturbances and flickering before the eyes
  • Pain in the right upper abdomen, as well as nausea and vomiting
  • Unilateral swelling of a leg with pain, tension, redness, or localized heat

Unilateral findings indicate an increased risk of thromboembolism, which is fundamentally elevated during pregnancy due to altered blood coagulation. Early investigation protects mother and child from severe complications.

When bending down is no longer possible without shortness of breath or pain, the care routine must be adapted. The principle of accessible care applies, utilising assistive devices and simplified procedures.

To relieve the veins and gently drain oedema, you should regularly integrate the following measures into your daily routine:

  • Elevate your legs several times a day for 15 to 20 minutes. A position where the feet are slightly above heart level is ideal. A wedge pillow or raised sofa cushions provide stable support.
  • Use long-handled aids for washing feet, such as a sponge with a telescopic handle, to avoid arduous bending in the shower.
  • Take foot baths at a lukewarm temperature, with a duration not exceeding ten minutes. Sea salt added to the water has a mild dehydrating effect on the upper skin layer.
  • Activate the calf muscle pump while sitting or lying down. Rotate your feet 15 times inwards and outwards, and alternately pull your toes firmly towards your shins and extend them downwards.
  • When buying shoes, ensure a wide toe box, a stable heel counter, and adjustable Velcro or lace fastenings that adapt to fluctuating swelling levels throughout the day.

A typical case study from podiatric practice illustrates the benefit of structured self-initiative. A patient in her 34th week of pregnancy complained of severely burning soles and painful pressure points on the forefoot. Inspection revealed a collapsed transverse arch and incipient calluses under metatarsal heads II and III. Another error involves trimming the nails. When the view of the toes is obstructed by the abdomen, many women blindly reach for nail scissors or clippers. In doing so, nail corners are often cut round to avoid bumping inside the shoe. Exactly this rounding causes the growing nail to pierce into the deeper tissue layers of the swollen nail fold. An ingrown nail should always be prevented by a straight transverse cut instead.

Wearing overly tight, cutting socks is another problem. Socks with tight elastic bands act as a constriction point at the ankles or calves. They interrupt lymphatic and venous return flow, which drastically worsens swelling in the foot area. Only medical compression stockings prescribed by a physician and precisely measured at a medical supply store, or vein-tested socks without cutting elastic, should be used.

Likewise, an increased shoe size tempts many women to switch to unpadded slip-ons, clogs, or slides without heel support. In such shoes, the toes perform a clawing motion with every step to keep the shoe on the foot. This leads to massive strain on the tendons of the toe flexors and extensors, promotes hammer toes, and additionally overloads the plantar fascia.

Finally, the indiscriminate use of over-the-counter, strongly diuretic tea blends or diuretics without medical advice is dangerous. Abrupt pharmaceutical or herbal fluid removal deprives the maternal vascular system of fluid, further thickening the blood and potentially compromising placental perfusion. Oedema treatment during pregnancy always belongs in the hands of trained medical personnel.

Current evidence and classification

The scientific literature convincingly demonstrates that changes to the foot during pregnancy are not a temporary phenomenon, but can have lasting structural consequences. A landmark study by Segal et al. investigated the morphological changes in the human foot during and after pregnancy. The researchers demonstrated that the collapse of the longitudinal arch and the increase in foot length persist in the majority of primiparous women even months after delivery [1]. The flattening of the arch leads to a permanent alteration in pressure distribution under the sole.

Regarding the treatment of pregnancy oedema, a meta-analysis of Cochrane reviews demonstrates that non-pharmacological interventions such as moderate water exercise, intermittent pneumatic compression, and wearing class I or II medical compression stockings can significantly reduce swelling [3]. Compression stockings increase tissue pressure from the outside, reducing fluid leakage from capillaries and promoting return flow.

Studies on the effect of manual lymphatic drainage and reflexology in pregnancy-related leg oedema also show positive effects on quality of life and circumference measurements [4]. Do contrast showers help against ankle swelling?

Contrast showers using cold and lukewarm water can stimulate venous tone and improve blood circulation. However, it is important that the water is not ice-cold, to prevent sudden vasoconstriction followed by dysregulation. Always start furthest from the heart at the feet and slowly direct the water jet upwards towards the thighs. Always finish with lukewarm water.

Are foot massages or lymphatic drainage safe during pregnancy?

Gentle effleurage strokes along the feet and lower legs, as well as professional manual lymphatic drainage, are very safe and effective for reducing oedema. Care should be taken to avoid massaging deep acupressure points that could reflexively trigger uterine contractions.
