The development of swelling in the area of the feet and ankle joints is based on a disruption of the physiological fluid balance in the tissue. Under normal conditions, the capillary vascular system continuously filters liquid blood components into the surrounding interstitium. This process is regulated by Starling forces, which balance the hydrostatic pressure within the capillaries against the colloid osmotic pressure of the blood. A functioning interplay of the venous system and lymphatic vessels ensures that extravasated fluid is reliably drained away.
The venous return of blood from the lower extremities against gravity requires significant mechanical work. The calf muscle pump occupies a central role here. With each flexion of the ankle joint, the gastrocnemius and soleus muscles compress the deep leg veins and propel the blood towards the heart. Venous valves function as non-return valves and prevent the backflow of the blood column. This mechanism is supported by the plantar venous plexus of the sole of the foot, which is compressed during each step by the heel-to-toe roll.
Simultaneously, the precapillary and postcapillary lymphatic system takes up the protein-rich tissue fluid that cannot be reabsorbed directly into the venules. When hydrostatic pressure in the capillaries rises, colloid osmotic pressure drops due to protein deficiency, or the transport capacity of the lymphatic vessels is impaired, fluid accumulates in the extracellular space. A clinically visible oedema usually only develops when the interstitial fluid volume has increased by at least ten to fifteen per cent.
The differentiation between venously induced oedemas and true lymphoedemas is based on the differing composition of the tissue fluid. Venous oedemas are initially characterised by a low protein content, whereas primary or secondary lymphoedemas are rich in macromolecules. Over time, this distribution leads to secondary structural changes in the connective tissue, such as an increase in collagen fibres and progressive induration of the subcutaneous tissue.
Typical signs
The presentation of swollen feet varies considerably depending on the underlying pathophysiology. A characteristic feature of venous oedema is an increase in swelling over the course of the day, with the ankle regions noticeably gaining volume. Affected individuals report a marked feeling of heaviness, cutaneous tension, and a sensation of tired legs after prolonged periods of sitting or standing. Applying pressure to the swollen area produces a visible indentation that recedes slowly only after several seconds or minutes.
An important diagnostic indicator is Stemmer's sign for differentiating a lymphoedema. If the fold of skin at the base of the second or third toe can no longer be pinched and lifted, or only with difficulty, this indicates manifest lymphoedema. In contrast, the skin usually remains pinchable in purely venous oedema. Lipoedema, by contrast, manifests as a symmetrical, painful proliferation of adipose tissue on the legs, with the dorsum of the foot remaining free of swelling.
With chronic persistent tissue pressure, dermal structures undergo lasting changes. Extravasated haemoglobin degrades in the tissue, leading to haemosiderin deposits. These manifest as brown to bluish pigmentations in the lower leg and ankle region. Left untreated, this condition leads to stasis dermatitis, epidermal thinning, and in late stages to atrophie blanche, characterised by porcelain-white, scarred skin areas.
Furthermore, impaired microcirculation favours the occurrence of hyperkeratosis and deep rhagades. The stretched tissue loses its natural elasticity, significantly weakening the barrier function of the skin. Fungal infections of the skin and nails find ideal growth conditions in moist, poorly perfused tissue folds. A trained eye recognizes these changes early and initiates appropriate measures.
Everyday causes
Inappropriate footwear represents one of the most common everyday causes of venous stasis. Shoes with rigid soles or elevated heels drastically restrict the natural heel-to-toe roll of the foot. As a result, the calf muscle pump is insufficiently activated because dorsiflexion in the upper ankle joint remains limited. The blood column remains in the distal vascular segments, continuously increasing capillary pressure.
Foot deformities likewise influence the haemodynamics of the lower extremity. In severe pes planovalgus, gait biomechanics and pressure distribution on the plantar venous networks change. Physiological venous compression during foot strike loses efficiency. Prolonged, immobile sitting or standing in professional life exacerbates this effect, as no active muscle pull counteracts gravity.
Adverse drug reactions are a widespread, frequently overlooked cause of peripheral swelling. Antihypertensive agents from the dihydropyridine calcium channel blocker class reduce precapillary vascular resistance while postcapillary tone remains unchanged. This leads to increased intracapillary pressure and enhanced fluid extravasation. Non-steroidal anti-inflammatory drugs, glucocorticoids, and hormonal preparations can also trigger significant fluid retention.
A practical example from podiatric practice illustrates these connections. A 62-year-old female patient presented with marked bilateral ankle oedema. Medical history revealed a change in medication to a calcium channel blocker several weeks prior, combined with the daily wearing of constrictive knee-high socks. Mechanical constriction at the level of the popliteal fossa significantly exacerbated the drug-induced capillary pressure. Following consultation with the attending physician and adjustment of the socks, the swelling subsided substantially.
When medical evaluation is required
Although swollen feet often have harmless causes, warning signs exist that necessitate immediate specialist evaluation. A sudden, unilateral swelling of the foot or lower leg must be classified as a suspected deep vein thrombosis until proven otherwise. If this condition is accompanied by pain, hyperthermia, bluish discoloration, or a sensation of tightness in the calf, a phlebologist or emergency department must be consulted immediately.
Systemic accompanying symptoms likewise demand swift action. If foot swelling occurs alongside acute dyspnoea, chest pain, tachycardia, or rapid weight gain of more than one and a half kilograms within 48 hours, acute decompensation of heart failure may be present. Signs of renal-induced oedema tendency, such as foamy urine or facial swelling, also require nephrological diagnostics.
Localised skin changes are also to be evaluated as critical. Rapidly progressing erythema with distinct margins, accompanied by fever and chills, points to erysipelas. This bacterial infection of the skin requires prompt antibiotic therapy. Open skin areas, weeping eczema, or blistering must never be treated self-sufficiently, as a high risk of infection exists.

Here are five effective measures that you can easily integrate into your daily routine:
- Performing venous gymnastics: Alternating between standing on tiptoe and standing on heels, as well as vigorously flexing and extending the toes while seated, directly activates the calf musculature.
- Wearing properly fitted compression garments: Medical compression stockings reduce the vascular diameter of the veins and increase blood flow velocity.
- Meticulous skin care with lipid-rich, urea-containing emulsions: A concentration of five to ten per cent urea strengthens the barrier function of the stretched epidermis and prevents micro-fissures.
- Avoiding extreme heat exposure: Refraining from hot baths, saunas, and direct sunlight, as warmth dilates the blood vessels and promotes fluid extravasation.
- Optimising fluid and salt intake: Adequate intake of still water combined with moderate reduction of dietary salt supports physiological electrolyte balance.
When selecting footwear, attention should always be paid to a flexible sole, a wide toe box, and a flat heel height. Heels over three centimetres prevent complete heel-to-toe roll of the sole and largely deactivate the muscle pump.
Common mistakes
In daily practice, we regularly observe improper practices that aggravate existing swelling or even cause secondary tissue damage. Many affected individuals resort to inappropriate home remedies when experiencing skin tension or neglect basic precautions.
You should strictly avoid the following five mistakes:
- Folding over or cutting into the top band of compression stockings: This creates a constricting ring that blocks venous return like an obstruction and provokes tissue damage.
- Using sharp rasps or blades for callus removal: With oedematous skin, the risk of injury is extremely high, which can lead to slow-healing wounds.
- Discontinuing prescribed medications on one's own initiative: Abruptly stopping diuretics or antihypertensive medications frequently leads to dangerous rebound effects in the cardiovascular system.
- Ignoring unilateral swelling or painful skin erythema: Hesitating when warning signs appear delays the diagnosis of potentially life-threatening conditions such as thrombosis.
- Excessive use of hot foot baths for relaxation: Thermal vasodilatation leads to increased plasma filtration into the tissue and rapidly worsens the oedema.
Another common problem is wearing socks with tight, elastic bands. These dig into the oedematous tissue and leave deeply indented marks. These constrictions not only impair cutaneous microcirculation, but also significantly increase the risk of local pressure ulcerations.
Current evidence and scientific context
The medical evidence regarding the treatment of leg and foot oedema highlights the necessity of cause-oriented, multimodal therapy. The current S2k guideline on medical compression therapy confirms that external pressure on the tissue reduces capillary pressure, increases tissue fluid reabsorption, and decreases venous diameter [1]. This leads to a significant acceleration of venous blood flow.
Studies on venous biomechanics show that the efficiency of the muscle-vein pump directly depends on the mobility of the upper ankle joint [2]. Do cool foot baths help with swollen ankles?

However, extremely cold ice baths should be avoided, as they can cause irritation to sensitive skin. In addition, it is essential to dry the feet thoroughly and gently after bathing to prevent skin maceration between the toes. If arterial circulatory disorders are present, cold applications must be avoided.
Can unsuitable footwear promote the formation of oedema?
Yes, unsuitable footwear is a common contributing factor to increased foot swelling. Shoes with rigid, inflexible soles or high heels prevent the foot from rolling off fully during walking. Consequently, the calf muscle-vein pump is not sufficiently compressed, which slows down venous return. Conversely, flat, well-fitting shoes with flexible soles support natural haemodynamics.
When should you consult a doctor immediately for swollen feet?
Immediate medical evaluation is required if the swelling occurs suddenly, unilaterally and in combination with pain, redness or localized warmth. These symptoms may indicate deep vein thrombosis or erysipelas. Simultaneous occurrence of shortness of breath, chest pain or extremely rapid weight gain also requires immediate medical attention.
