What lies behind it
In everyday language, the terms podiatry and cosmetic foot care are frequently used synonymously. However, fundamental differences exist from a legal, educational and medical perspective. In Germany, the right to use the professional title Podologin or Podologe (podiatrist), or medizinische Fußpflegerin or medizinischer Fußpfleger (medical foot care specialist), has been protected by the Podiatrists Act (Podologengesetz) since 2002. Anyone wishing to use this title must complete a two-year full-time training course or a three-year part-time training course at a state-recognised school and successfully pass the state examination.
Podiatric training comprises at least 2,000 hours of theoretical and practical instruction as well as 1,000 hours of practical training in specialised clinics and podiatry practices. The curriculum encompasses anatomy, physiology, special pathology, microbiology, hygiene, dermatology, diabetology and orthopaedics. Podiatrists are recognised healthcare professionals. They work as partners to physicians and perform independent curative treatments on the foot, provided a medical prescription is present or an additional qualification as a sectoral practitioner (sektorale Heilpraktikerin) is held.
Cosmetic foot care, by contrast, falls under trade law and is not a state-regulated occupation. Qualifications are often acquired through multi-day or weekend courses at private training facilities, without state verification of knowledge. Legally, cosmetic foot care providers are strictly limited to performing care measures on the skin and nails of healthy feet. As soon as signs of disease, skin alterations, pain or systemic risk factors are present, cosmetic foot care exceeds its legal remit. Practising medicine without the corresponding licence or permit violates the Non-Medical Practitioners Act (Heilpraktikergesetz).
Another central difference concerns hygiene regulations. Podiatry practices are subject to the strict requirements of the Infection Protection Act and the guidelines of the Robert Koch Institute. The reprocessing of instruments requires documented thermal disinfection and sterilisation in Class B vacuum autoclaves. Medical devices must be fully traceable. Lower regulatory hygiene requirements typically apply to cosmetic foot care, as no invasive or skin-penetrating procedures are permitted in that setting.
Typical signs
In everyday life, it is not always easy for individuals to determine whether a foot is considered healthy or already requires medical treatment. Typical signs indicating podiatric intervention can be found on both the nails and the skin of the foot. Ingrown nails, medically termed unguis incarnatus, are usually accompanied by redness, swelling and severe pressure pain in the lateral nail wall. If granulation tissue has already formed, commonly known as proud flesh, explicit tissue damage is present.
Thickened, discoloured or brittle nail plates frequently indicate onychomycosis. This fungal infection of the nail plate requires the professional debridement of the infected nail substance using medical burs in order to reduce the fungal burden and enable topical therapeutics to act effectively. Severely curved nails, such as those occurring in onychogryphosis, can cause severe pain when walking due to massive pressure on the nail bed.
On the skin, podiatric conditions usually manifest as excessive keratinisation, known as hyperkeratosis. This develops as a protective response by the body to chronic pressure and friction. When these callus layers grow deep into the dermis, corns form, scientifically referred to as clavi. The central keratin plug presses directly onto sensitive nerve endings in the dermis, triggering sharp pain. Painful heel fissures, known as rhagades, which can extend into bleeding and pain-sensitive skin layers, also represent a clear medical problem.
Particular caution is required when systemic diseases are present. People with diabetes mellitus, peripheral arterial disease or rheumatic conditions frequently exhibit sensory disturbances or circulatory disorders. A loss of protective sensation means that minor injuries, pressure points or foreign objects in the shoe are not noticed in time. In these cases, every foot is precautionary classified as a high-risk foot and should be managed exclusively by trained podiatrists.
Cosmetic foot care, on the other hand, is purely preventive and decorative. It comprises the professional shortening of healthy nails, filing nail edges, removing mild physiological calluses without using scalpel blades, and applying care products or nail polish. As soon as the objective is pain relief or the removal of pathological tissue changes, the scope of cosmetic foot care is no longer sufficient.
Everyday causes
Painful foot conditions rarely arise spontaneously. They are usually the result of years of improper mechanical loading, unsuitable footwear, underlying systemic diseases or incorrect care methods. Wearing shoes that are too tight, too pointed or too high forces the toes into an unnatural position. Continuous pressure on the sides of the toes promotes the ingrowing of the nail edges into the sensitive nail wall. In addition, ill-fitting footwear leads to localised pressure peaks on the soles of the feet and the toe joints.
Anatomical foot deformities such as splayfoot, fallen arches (pes plano-valgus) or hallux valgus alter the biomechanics of the entire musculoskeletal system. The collapse of the foot arch shifts the primary load-bearing points on the sole of the foot. As a physiological protective response, the skin produces increased keratin at these sites. If pressure relief is not provided, the callus continues to build up, loses its elasticity and tears painfully under tensile stress.
One of the most significant internal medical causes of foot complications is diabetes mellitus. Permanently elevated blood glucose levels damage both the microvasculature and the peripheral nerve pathways. Diabetic autonomic neuropathy causes a reduction in sweat secretion. The skin becomes extremely dry and cracked, losing its natural protective function against mechanical pathogens. At the same time, sensory neuropathy impairs pain perception, eliminating pain as the body's natural warning signal.
Peripheral arterial disease reduces arterial blood supply to the tissues. Reduced perfusion leads to a decreased supply of oxygen and nutrients to the skin and nails. The tissue becomes vulnerable, nail plates grow more slowly and thicken, while wound healing is drastically impaired. Under such conditions, even a minute lesion can lead to the development of a chronic ulcer.
Common mistakes in home foot care further exacerbate these risks. Cutting toenails in a rounded shape deep into the lateral nail folds is a primary cause of ingrown nails. As the nail grows back, the sharp cut edge acts like a rail, pushing into the adjacent soft tissue. Improper use of over-the-counter callus shavers or aggressive chemical callus removers also routinely damages healthy epithelial tissue.
When medical evaluation is necessary
Podiatrists work in close collaboration with physicians to ensure comprehensive patient care. Certain clinical red flags require immediate evaluation by a general practitioner, diabetologist, dermatologist or vascular specialist. Findings involving acute symptoms of infection demand urgent attention.
Pronounced erythema that feels warm to the touch and spreads beyond the immediate focus of inflammation suggests a bacterial tissue infection. If pus exudes from the nail fold or a skin wound, targeted microbiological investigation and, if necessary, systemic antibiotic therapy must be initiated. A rapidly ascending red streak on the skin can indicate lymphangitis, which requires immediate medical treatment.
Open wounds, ulcers or deep tissue defects on the feet always represent a medical emergency. This applies particularly to individuals with diabetes. Wounds occurring as part of diabetic foot syndrome carry a high risk of becoming chronic and present a significant amputation risk without prompt medical and podiatric wound management. If tissue turns dark or black, tissue necrosis or gangrene is present, requiring immediate medical intervention.
Sudden changes in skin colour, such as a pale or cyanotic purplish discolouration of the toes combined with cold skin and rest pain, also necessitate a vascular evaluation of arterial circulation. As part of their diagnostic assessment, podiatrists perform preliminary examinations such as pulse checks of the dorsalis pedis and posterior tibial arteries, alongside sensory testing. If abnormal findings emerge, treatment is suspended, and the patient is referred directly to the attending physician.
What podiatric treatment can achieve
The spectrum of podiatric services is diverse and grounded in sound medical methods. Every initial consultation begins with a detailed medical history, systematically recording pre-existing conditions, medication, allergies and current symptoms. This is followed by visual and palpatory inspection of the skin and nails, as well as testing of protective sensation using a Semmes-Weinstein monofilament and a Rydel-Seiffer tuning fork.
The podologische Komplexbehandlung (comprehensive podiatric treatment) includes the professional trimming and filing of toenails, probing and cleaning of nail folds, and reduction of thickened nails. For pain relief in cases of ingrown nails, nail brace technique, also known as orthonyxia, is frequently employed. An individually fitted brace made of spring steel wire or plastic is fixed onto the nail. It gently lifts the lateral nail edges out of the inflamed fold and corrects nail growth sustainably, without requiring surgical intervention.
Skin alterations such as excessive keratinisation are precisely and painlessly removed layer by layer using sterile scalpel blades. Corns are carefully enucleated from deep within the tissue using specialised gouge blades or rotary burs. To relieve pressure and correct deformities, podiatrists manufacture custom orthoses from durable medical-grade silicone materials. A key difference compared to cosmetic foot care concerns reimbursement through statutory and private health insurance funds. The duration of a podiatric treatment ranges between 30 and 60 minutes, depending on the extent of the clinical findings. Treatment intervals depend on the individual condition. For patients with diabetes who present a high-risk profile, treatments are usually carried out at intervals of four to six weeks to identify and prevent complications at an early stage.
What you can do yourself
Consistent and appropriate foot care at home significantly supports medical treatment provided in the practice. Individuals should establish a daily care routine tailored to the specific needs of their skin.
- Perform a thorough visual inspection of your feet daily. Look for redness, small cuts, blisters or scaling skin. If necessary, use a hand mirror or ask family members for assistance.
- Wash your feet daily with lukewarm water. The water temperature should not exceed 37 degrees Celsius. Footbaths should be limited to a maximum of three minutes to prevent softening of the skin layers.
- Dry your feet thoroughly after washing. Pay particular attention to the spaces between the toes. Moisture in these areas promotes the growth of skin fungi and bacteria.
- Apply a moisturising cream to the soles of your feet and heels daily. Care products containing a urea concentration of 10 to 15 per cent are recommended. Avoid applying cream between the toes.
- Always cut your nails straight across. Use toenail clippers or a file. The corners of the nails should not be rounded off or cut deep into the nail fold. Remaining sharp edges can be smoothed with a glass file.
- Before putting on your shoes, inspect them for foreign objects, pressing seams or bunched lining. Wear socks made of natural fibres without constricting elastic bands or noticeable seams in the toe area.
Common mistakes
In daily practice, the same typical mistakes in home foot care recur regularly, triggering painful complications. By far the most common mistake is cutting toenails into a rounded shape or clipping out the lateral corners. Many individuals attempt this to relieve a slight feeling of pressure in the nail fold. Removing the corner of the nail allows soft tissue to expand into the space. As the nail grows back, it meets this tissue barrier and digs deep into the skin, resulting in chronic and extremely painful inflammation of the nail wall.
Another dangerous mistake is using sharp tools for callus removal at home. Callus shavers, razor blades or electric grinding tools carry a significant risk of injury. If a blade slips, it injures not only the callus but also the underlying healthy skin deeply. Bacteria can enter unhindered, potentially causing severe phlegmons or abscesses.
Prolonged and hot footbaths are mistakenly regarded as beneficial. Extended soaking causes the keratin in the stratum corneum to swell excessively, leaching out the natural protective lipids of the skin barrier. Consequently, the skin loses its mechanical resilience and becomes prone to even greater keratinisation and cracking.
The use of over-the-counter corn plasters is also frequently misused. These plasters contain highly concentrated salicylic acid intended to dissolve keratin tissue. If the plaster is applied imprecisely, the acid burns the surrounding healthy tissue, creating deep chemical burns and painful wounds that heal very slowly, particularly in high-risk patients.
Ignoring skin changes between the toes is another widespread error. Mild redness, itching or white skin maceration are often dismissed as insignificant. However, these frequently represent tinea pedis. Left untreated, this fungal infection damages skin integrity, creating a portal of entry for streptococcal bacteria that can trigger life-threatening erysipelas.
Current evidence and medical context
The medical necessity of professional podiatric care in high-risk patients is supported by numerous scientific studies and clinical practice guidelines. In the German National Care Guideline for Type 2 Diabetes (Nationale VersorgungsLeitlinie Typ-2-Diabetes), podiatric prevention plays a central role in preventing foot ulcerations and lower leg amputations [1]. Studies demonstrate that integrating qualified podiatrists into interdisciplinary diabetes care can reduce amputation rates in high-risk patients by 50 to 60 per cent [2].
Research evaluating the efficacy of nail brace therapy demonstrates that orthonyxia achieves success rates comparable to partial surgical resection in mild to moderate ingrown toenails [3]. At the same time, this conservative podiatric method offers the advantage of preserving tissue, avoiding downtime and eliminating postoperative infection risks. When applied correctly alongside pressure relief from suitable footwear, the recurrence rate is significantly lower than that associated with wedge resection.
Dermatological studies examining skin barrier function confirm the benefit of topically applied moisturising factors. Preparations containing a 10 per cent urea concentration lead to a measurable reduction in keratinocyte hyperproliferation with regular use and significantly improve skin elasticity in diabetic patients [4]. This underscores the importance of adjunctive topical care within podiatric therapy.
Epidemiological data also support the necessity of stringent hygiene and reprocessing standards in podiatry practices. Studies on mycoses and bacterial nosocomial infections demonstrate that improperly cleaned rotary instruments and reused files can act as vectors for fungal spores and spore-forming bacteria [5].
