What lies behind it

The human nervous system conducts signals such as touch, temperature, vibration, and pain from the periphery of the body to the spinal cord and onwards to the brain. The nerve fibres responsible for sensation in the feet extend from the lumbar vertebrae through the legs down into the smallest branches of the toes. Medically, various types of sensory disturbances are distinguished. Paraesthesias are non-painful abnormal sensations such as tingling, formication, or prickling. Hypaesthesia refers to a reduction in normal sensitivity to tactile or painful stimuli. Anaesthesia describes the complete loss of any sensation within a defined area.

Different types of nerve fibres are responsible for transmitting sensory impressions. Thick, myelinated A-beta fibres conduct tactile and vibratory stimuli extremely quickly. Thinner A-delta fibres and unmyelinated C-fibres are responsible for pain and temperature perception. When peripheral nerves are damaged by sustained pressure, inflammatory processes, or nutrient deficiencies, signal transmission becomes disrupted. The brain interprets these faulty or absent impulse sequences as tingling, burning, or numbness.

Beside the nerves, the microcirculation plays a central role. The finest blood vessels, the vasa nervorum, supply the nerve pathways with oxygen and glucose. If this blood flow is restricted by vascular constriction or atherosclerotic plaques, the nerve cells suffer from oxygen deprivation. The nerves gradually cease to function. Impaired blood circulation thus exacerbates the sensory deficit and delays regenerative processes in the tissue.

Typical signs

Symptoms of sensory disturbances in the feet usually develop insidiously. The complaints often begin at the tips of the toes and subsequently spread symmetrically across the entire forefoot. Specialists refer to this pattern as a stocking-like distribution of sensory loss. Affected individuals frequently report feeling as though they are walking on cotton wool, on sand, or as if they are constantly wearing socks that are too thick. A typical sign is also the inability to feel fine unevenness in the ground with the soles of the feet.

Alongside numbness alone, burning pain frequently occurs, intensifying particularly during evening and night hours. In neurology, this phenomenon is referred to as burning feet syndrome. The sensation of having cold feet even though the skin feels warm to the touch also indicates faulty neurological signal processing. Conversely, genuinely ice-cold feet with pale or bluish skin discoloration can indicate a purely circulatory disorder.

A clear distinction between nerve-related and vascular causes is essential. In arterial circulatory disorders, pain occurs primarily during physical exertion, such as brisk walking, forcing the individual to stop. Neuropathic complaints, by contrast, manifest predominantly during periods of rest and complete offloading of the legs. A careful examination of the timeline and exact localisation helps to narrow down the underlying problem.

A patient from the district of Unterallgäu described in our practice that she only noticed the hot water during a foot bath when the skin on the dorsum of her foot was already severely reddened. Such accounts are classic indicators of the loss of protective thermal sensation. When hot or cold stimuli are no longer perceived correctly, the risk of injury in daily life increases drastically.

Causes in daily life

Sensory disturbances in the lower legs and feet can have diverse triggers. In daily life, mechanical compressions play a significant role. Wearing footwear that is too tight or inflexible exerts continuous pressure on the interdigital nerve pathways. In tarsal tunnel syndrome, the tibial nerve is compressed as it passes through the osseous canal behind the medial malleolus. This leads to pain and numbness on the sole of the foot, which can radiate into the toes.

Alterations in the spine represent another common cause. A herniated disc in the lumbar spine or a narrowing of the spinal canal can compress the nerve roots of segments L4 to S1. This root irritation projects abnormal sensations precisely into the innervation area of the affected nerve in the foot. Foot deformities such as pronounced splayfoot or hallux valgus also alter pressure distribution during gait rollover, leading to local nerve irritation.

Among systemic underlying diseases, diabetes mellitus ranks first. Chronically elevated blood sugar levels damage both the microvasculature and the nerve structures directly. Diabetic polyneuropathy develops, which, left untreated, leads to a complete loss of pain perception. Chronic alcohol consumption, kidney disease, or hypothyroidism also cause lasting damage to peripheral nerves.

An often overlooked factor is vitamin B12 deficiency. This vitamin is essential for the formation of the myelin sheath, which insulates nerve fibres. A long-term deficiency, caused by chronic gastrointestinal diseases, a strictly vegan diet without supplementation, or long-term intake of certain medications such as metformin, leads to degeneration of the posterior columns of the spinal cord and peripheral nerves.

When medical evaluation is necessary

Not every brief sensation of tingling requires immediate alarm. If a foot falls asleep after prolonged sitting with crossed legs and quickly returns to normal after standing up, it is merely a temporary interruption of conduction. However, if sensory disturbances persist or increase in intensity, structured medical diagnostics are indispensable.

There are clear red flag signs that require immediate consultation with a physician or emergency department. These symptoms indicate acute, potentially dangerous disorders of the nervous system or blood vessels:

  • Sudden numbness in a leg or foot, particularly in combination with paralysis or weakness when lifting the toes
  • Sudden loss of sensation together with speech disturbances, visual disturbances, or dizziness, which can indicate a stroke
  • Acute, severe pain in the leg associated with paleness, coldness of the skin, and an absent pulse in the dorsalis pedis artery, suggesting an acute arterial occlusion
  • Visible wounds, blisters, or dark skin changes on the foot that you did not feel yourself and that cause no pain
  • Rapid progression of sensory disturbance within a few days or spreading of symptoms to the hands

Early evaluation by neurologists, diabetologists, or vascular specialists prevents irreversible damage to nerve structures. The earlier the underlying condition is diagnosed and managed, the higher the chances of stabilizing or even reversing the abnormal sensations.

What podiatric treatment can achieve

Podiatry plays a key role in the prevention, early detection, and professional care of patients with sensory disturbances. As a healthcare profession, podiatry collaborates closely with treating medical specialists. In our practice, every initial consultation begins with a comprehensive podiatric medical history and a thorough inspection of the feet.

To assess sensitivity, we utilize scientifically evaluated testing procedures. Using a Semmes-Weinstein monofilament with a defined buckling force of exactly ten grams, we test tactile and protective sensation at specific points on the sole of the foot. If the patient can no longer perceive the light pressure of the synthetic fibre, there is a relevant risk of loss of protective sensation. We test vibration sensitivity using a 128 Hz Rydel-Seiffer neurological tuning fork on bony landmarks such as the first metatarsophalangeal joint and the malleoli. To evaluate thermal sensation, a specialized instrument with a cold metallic side and a warm plastic side is used.

In addition, we evaluate the peripheral vascular status by palpating the foot pulses, namely the dorsalis pedis artery and the posterior tibial artery. If findings show abnormalities, we arrange targeted referral to appropriate medical specialists. Establish a consistent daily routine to protect your feet from damage. The following measures will help you effectively minimize the risk of injury:

  • Inspect the soles of your feet, heels, and spaces between your toes carefully every evening for redness, pressure spots, blisters, or small cuts. Use an unbreakable hand mirror if necessary or ask family members for assistance.
  • Before putting on your shoes, reach inside with your hand every time to check for foreign objects such as small stones, pressing seams, or folded insoles.
  • Wash your feet daily with lukewarm water and a mild, soap-free cleansing lotion. Always check the water temperature with a bath thermometer or your forearm to prevent burns.
  • Dry your feet thoroughly after washing, especially between the toes, to prevent fungal infections.
  • Care for your skin daily using a moisturizing, urea-containing foot cream or foam with a urea content of five to ten percent. Avoid applying cream between the toes, as excessive moisture can macerate the skin there.
  • Always wear socks without pinching seams and with a soft band that does not restrict blood circulation. Prefer light-coloured socks made of cotton or wool, as wound fluid or blood becomes immediately visible on them.
  • Never walk barefoot, whether indoors or outdoors. Even minor uneven surfaces, wood splinters, or spilled liquids can lead to severe tissue damage unnoticed.

Pay attention to selecting optimal footwear as well. Shoes must offer sufficient space for the feet without rubbing. Purchase new shoes preferably in the late afternoon, as feet tend to swell slightly during the day. The sole should be flexible yet provide adequate protection against penetrating objects.

Common mistakes

In podiatric practice, we repeatedly observe well-intentioned yet dangerous care errors by patients. The most frequent mistake is prolonged, hot foot baths. Individuals with numbness often complain of ice-cold feet and attempt to warm them using very hot water. Due to impaired temperature perception, they fail to notice that the water is far too hot. This results in severe second- or third-degree burns that heal extremely slowly.

Another serious error is attempting to remove hyperkeratosis, corns, or pressure spots independently using sharp instruments. Using razor blades, corn shavers, or nail scissors almost inevitably leads to cuts when sensitivity is reduced. Because the wound often does not hurt due to nerve damage, it receives inadequate care. Pathogens enter unhindered, triggering severe infections.

Over-the-counter corn plasters also represent a significant source of danger. These plasters contain highly concentrated salicylic acid intended to chemically dissolve hyperkeratotic tissue. On thin, poorly perfused, or neuropathic skin, the acid penetrates deeply into healthy tissue. Chemical burns and deep tissue defects develop, requiring months of treatment.

Cutting toenails deep into the nail folds is another typical management mistake. Rounded nail corners tend to ingest into the skin as they regrow. Painful inflammation and granulation tissue develop. Toenails should always be trimmed straight across, with the corners filed gently.

Finally, many affected individuals wait too long before seeking help. Ignoring subtle abnormal sensations allows neurodegenerative processes to progress unchecked. Early diagnosis and regular professional foot care offer the best protection against permanent functional impairments.

Current evidence and classification

Scientific literature strikingly demonstrates the close association between peripheral sensory disturbances and the risk of severe lower extremity complications. The German National Care Guideline for Type 2 Diabetes summarizes that diabetic neuropathy is the strongest single risk factor for developing foot ulceration [1]. Studies indicate that loss of perception of the 10-gram monofilament increases the risk of developing a foot wound sevenfold.

Several multicentre studies have investigated how effectively preventive structural measures can reduce secondary damage. Systematic refinement of screening combined with regular podiatric care can lower amputation rates in high-risk patients by 50 to 80 percent [2]. Regular professional debridement of hyperkeratosis demonstrably reduces peak plantar pressures, serving as a primary protective mechanism against wound formation [3].

Regarding diagnostic accuracy, neurological societies emphasize combining multiple testing procedures. The sole use of a monofilament is often insufficient to detect early stages of small-fibre neuropathy. Only the combination of monofilament, tuning fork testing, and thermal sensitivity testing provides high sensitivity for detecting peripheral nerve damage [4].

Research in vascular medicine further proves that microvascular dysfunctions correlate closely with neuropathic symptoms. Reduced capillary perfusion leads to local hypoxia in nerve tissue, slowing the propagation of action potentials [5]. Continuous monitoring of peripheral circulation and skin integrity by trained specialists is therefore evidence-based and medically necessary.

What is the difference between neuropathy and a circulatory disorder?

Neuropathy involves direct damage to nerve fibres, leading to abnormal sensations, burning, and numbness, often primarily at rest. A circulatory disorder is a narrowing of the blood vessels, resulting in insufficient oxygen reaching the tissue. Vascular pain typically occurs under exertion, and the legs appear pale and cold. However, both conditions can also occur simultaneously.

How often should you see a podiatrist for sensory disturbances?

If sensory impairment is confirmed, a treatment interval of four to six weeks is recommended. This schedule enables the podiatrist to remove calluses in good time before dangerous pressure builds up on the underlying tissue. In addition, fresh skin changes or microtrauma can be detected and treated at an early stage. Acute pressure spots may require shorter follow-up intervals.

Does health insurance cover podiatric treatment for numb feet?
