What lies behind it

A nocturnal calf cramp, medically termed crampus suralis, is an involuntary, painful and sustained contraction of the lower leg musculature. The primary muscle involved is the triceps surae muscle, which consists of the two-headed gastrocnemius muscle and the underlying soleus muscle. This muscle group merges into the Achilles tendon (tendo calcaneus) and transmits its force directly onto the calcaneus. The plantar aponeurosis on the sole of the foot forms a functional continuation of this structure. Due to this anatomical link, abnormal tension in the foot interacts directly with tension in the lower leg.

At a biochemical level, muscle contraction arises through the interplay of nerve impulses and calcium ions. When an action potential arrives at the motor endplate of the muscle via an alpha motor neuron, calcium is released from the sarcoplasmic reticulum into the cytosol of the muscle cell. Calcium enables the sliding mechanism between the protein structures actin and myosin. Adenosine triphosphate (ATP) is required to subsequently detach this bond. In this process, magnesium acts as a natural antagonist to calcium. It regulates the inflow pathways at the ion channels and dampens the excitability of the neural membrane. If magnesium is lacking or the electrolyte ratio shifts, the actin-myosin cross-bridges remain locked in place: the muscle cramps.

During sleep, the development of cramps is favoured because proprioceptive control is downregulated. The stretch reflex system, consisting of muscle spindles and Golgi tendon organs, operates at a low level of sensitivity during rest. If the foot rests in a slightly downwardly flexed position (plantarflexion) in bed, the gastrocnemius muscle shortens passively. In this approximated position, even a minimal aberrant discharge of the alpha motor neuron is sufficient to trigger a maximum contraction, which the body can no longer autonomously inhibit via the regulating tendon organs during sleep.

Typical signs

The primary cardinal symptom of a nocturnal calf cramp is an abruptly occurring, severe pain in the lower leg or sole of the foot. Sufferers wake up abruptly. On palpation, the affected musculature appears board-hard, nodularly indurated and extremely sensitive to pressure. Not infrequently, the toes remain in a fixed flexed position during the episode, pointing to the involvement of the long flexor muscles of the toes (flexor hallucis longus and flexor digitorum longus). An acute episode usually lasts from a few seconds to several minutes. After the actual contraction subsides, a dull feeling of pressure often remains for many hours, resembling muscle soreness and caused by microscopic overstretching of the muscle fibres.

Precise classification requires differentiation from other neurological and vascular conditions. Restless legs syndrome (RLS), for instance, does not manifest as painful muscle contractions, but rather as an unbearable urge to move the legs, occurring primarily during periods of rest and accompanied by abnormal sensations such as tingling or pulling. Movement immediately improves RLS symptoms, whereas a true cramp can be intensified by active muscle contraction.

Similarly, peripheral arterial disease (PAD) must be ruled out. PAD leads to ischaemic pain that occurs primarily under exertion and rapidly subsides upon stopping, often referred to as intermittent claudication. Diabetic polyneuropathy also manifests differently. It presents predominantly with symmetrical numbness, burning pain (burning feet syndrome) or reduced pain and temperature sensation in the toes, without palpable muscle induration necessarily being present.

Causes in everyday life

The triggers for nocturnal calf cramps are diverse and often lie in a combination of static abnormal loading, inadequate fluid intake and overuse. Altered foot posture is among the most frequently overlooked factors. If flexible flatfoot (pes planovalgus) is present, the medial longitudinal arch collapses during weight-bearing. The heel tilts outwards, leading to continuous rotation of the lower leg. To compensate for this instability, the calf musculature must perform increased stabilising work with every step. By evening, the muscle is exhausted and reacts during the nocturnal rest phase with uncontrolled hypercontractions.

Unsuitable footwear also plays a significant role. Harmful options include shoes with an inflexible sole or an elevated heel. When wearing high heels, the calf musculature remains in a shortened position for hours. The body adapts to this prolonged posture through shortening of tendon structures and reduced fascial elasticity. When these shoes are taken off in the evening and the feet are stretched out in bed, the shortened musculature comes under mechanical tension that promotes the cramp reflex.

Further lifestyle-related causes include:

  • Chronic fluid deficit and lack of hydration, leading to a reduction in intracellular volume.
  • Excessive athletic strain without subsequent reconditioning and adequate recovery periods.
  • Prolonged sitting with bent legs or feet resting on the tiptoes, which impedes blood circulation.
  • Use of overly heavy or tightly tucked duvets that press the feet into plantarflexion throughout the night.

Alongside these factors, systemic conditions also exert an influence. During pregnancy, hormonal changes alter venous tone and markedly increase magnesium requirements. In older age, natural muscle mass decreases (sarcopenia) while tendons lose elasticity. This increases the susceptibility of muscle spindles to dysregulation.

When medical evaluation is necessary

Although nocturnal calf cramps are harmless in most cases and stem from functional disorders, clear warning signs exist that necessitate prompt medical diagnostics. If cramps abruptly increase in intensity, occur multiple times per night or disrupt sleep over weeks, a medical examination should be conducted. Metabolic conditions such as diabetes mellitus, thyroid dysfunction (hypothyroidism) or renal impairment must be excluded in this context.

Particular attention is required if the cramps are accompanied by unilateral swelling, redness or localized warmth in the lower leg. These symptoms can indicate deep vein thrombosis (DVT). A clot in the deep veins represents a life-threatening situation due to the risk of pulmonary embolism. Visible skin changes, parchment-like skin or poorly healing wounds around the ankle area also urgently require investigation of the venous and arterial systems.

Neurological deficits likewise require specialist medical evaluation. These include persistent numbness, muscle atrophy of the lower leg, or foot drop where the tip of the foot can no longer be raised properly during walking. Finally, prescribed medication should be reviewed. Drugs such as diuretics for hypertension, statins for cholesterol reduction, beta-blockers or certain asthma medications are suspected of inducing electrolyte shifts or triggering direct myopathies.

What podiatric treatment can achieve

Podiatry considers the foot to be the foundation of the entire kinetic chain. Care in a specialist clinic begins with a detailed medical history and a biomechanical assessment.   1. Stretching the calf musculature before going to bed: Stand in front of a wall with one leg placed backwards. The rear leg remains fully extended with the heel firmly on the floor. Slowly push the pelvis forward until a distinct but painless tension is felt in the calf. Hold this position for at least 30 seconds without bouncing. Repeat the exercise with the rear knee slightly bent to specifically stretch the soleus muscle.
  2. Stretching the sole of the foot: Sit down, place the foot onto the thigh and gently pull the toes towards the shin with your hand. This puts the plantar aponeurosis under stretch.
  3. Self-massage with a fascia roller or tennis ball: While standing or sitting, slowly roll the sole of the foot from the ball of the toes to the heel using gentle pressure. This releases local adhesions in the connective tissue.
  4. Adjusting sleep position: Ensure that the feet are not permanently extended downwards in bed. Side sleepers can place a pillow between their legs. Front sleepers should let their feet hang over the edge of the mattress to allow a neutral 90-degree position at the ankle joint.
  5. Balanced fluid and electrolyte intake: Drink sufficient water or unsweetened tea throughout the day. Ensure a diet rich in magnesium and potassium through nuts, whole grain products, legumes and bananas.

During an acute episode: remain calm. The most effective immediate measure is passive stretching. Grasp the tips of the toes on the affected leg and pull them firmly towards the shin while keeping the knee fully extended. Alternatively, place the foot flat on the cold floor and shift body weight onto the affected leg. The stretch stimulus activates the Golgi tendon organs, which send an immediate relaxation signal to the spinal cord.

Common errors

Typical behavioural patterns regularly manifest in practice that unintentionally exacerbate the problem or delay recovery. The most common error lies in the uncontrolled high-dose intake of magnesium supplements without prior diagnostic evaluation. Many sufferers assume that a magnesium deficiency underlies every cramp. Better absorption rates or underlying biomechanical blockages are ignored in the process. Furthermore, an overdose can lead to gastrointestinal distress and diarrhoea, which further worsens electrolyte balance through the loss of potassium and sodium.

Another serious error is working forcefully into the acute cramp through uncontrolled stamping or sudden jerking of the foot. Current state of research and classification

The scientific evidence base on nocturnal calf cramps has become considerably more nuanced in recent years. For a long time, oral administration of magnesium was considered standard therapy. However, a comprehensive Cochrane systematic review by Garrison et al. demonstrated that magnesium supplementation in older adults without a proven severe deficiency produces no statistically significant reduction in cramp frequency or intensity compared to placebo [1]. Moderate evidence for the benefit of magnesium was shown only in pregnant women. This underscores the necessity not to narrow therapeutic approaches purely to metabolic supplements.

Conversely, clinical studies demonstrate the high efficacy of physiotherapeutic and manual interventions. A randomised controlled trial by Hallegraeff et al. investigated the effect of daily stretching exercises of the calf and thigh musculature before bedtime in older patients [2]. Does magnesium help immediately against nocturnal calf cramps?

No, taking magnesium during an acute cramp does not produce an immediate effect, as gastrointestinal absorption takes hours. In cases of a proven deficiency, magnesium serves medium to long-term prophylaxis.
