Underlying Pathophysiology

The lymphatic system is an essential component of the human immune and fluid transport systems. In a healthy organism, initial lymphatic capillaries absorb the protein-rich interstitial fluid that cannot be reabsorbed into the bloodstream via the venous capillaries. Via pre-collectors and collectors, this interstitial fluid is transported towards the major lymphatic trunks by rhythmic contractions of the lymphangions. If mechanical insufficiency of the lymphatic system occurs, this drainage is permanently impaired.

Fluid and high-molecular-weight plasma proteins accumulate in the interstitial tissue. This protein-rich oedema fundamentally distinguishes lymphedema from low-protein oedemas, such as those occurring in isolated venous diseases or heart failure. The persistent high protein content in the extracellular space triggers chronic inflammatory reactions. Consequently, fibroblasts are stimulated to produce increased connective tissue. This process leads to tissue induration, known as fibrosclerosis.

In the feet, this tissue alteration manifests particularly prominently. A distinction is made between primary and secondary forms of lymphedema. Primary lymphedema is based on congenital malformations of the lymphatic vessel system, such as hypoplasia or aplasia of the vessels. Secondary lymphedema, by contrast, develops due to external influences such as surgical procedures, lymph node dissections, radiotherapy, trauma or prior inflammation.

Chronic swelling significantly alters the architecture of the skin surface. The epidermis responds to altered pressure and nutritional conditions with accelerated cell division. This results in hyperkeratosis, meaning severely hardened callosities, and in advanced stages, papillomatosis cutis lymphostatica. In this condition, wart-like, cobblestone-like skin growths form, in whose depressions moisture and pathogens can accumulate.

Typical Signs

The cardinal diagnostic sign for clinical diagnosis of lymphedema in the foot is Stemmer's sign, also known as the Kaplan-Stemmer sign. To evaluate this sign, the examiner attempts to pinch and lift a fold of skin over the dorsal aspect of the second or third toe between the thumb and index finger. If the skin fold cannot be pinched and lifted, or only with considerable effort, Stemmer's sign is considered positive. A positive Stemmer's sign is a highly specific indicator of lymphedema.

In addition to Stemmer's sign, other characteristic alterations appear in foot anatomy. These include the box-shaped transformation of the toes, medically referred to as squared toes or box toes. The individual toes lose their rounded contour and appear cuboid or flattened due to taut oedematous swelling. The natural skin creases over the metatarsophalangeal and interphalangeal joints smooth out completely or deepen into profound, hard-to-reach skin furrows.

The dorsum of the foot typically exhibits a taut, cushion-like swelling. The malleolar structures lose their distinct contour and blend into the surrounding tissue. While the swelling in early stages (Stage I) can still be pitted by digital pressure and partially regresses upon elevation, the tissue increasingly hardens in Stage II. Pitting becomes virtually impossible, and elevation no longer leads to substantial relief.

Differentiating lymphedema from other forms of oedema is crucial for selecting the correct care strategy. Lipedema always occurs symmetrically on both legs but completely spares the feet and hands. Stemmer's sign is always negative in lipedema, and marked tenderness to pressure is present. Phlebological oedema, caused by chronic venous insufficiency, usually manifests around the ankles, is accompanied by brownish skin hyperpigmentation from haemosiderin deposits, and typically yields a negative Stemmer's sign at the toes.

Everyday Causes

In daily life, numerous factors negatively influence the course of lymphedema in the foot and increase the risk of tissue damage. Wearing inappropriate footwear plays a central role. Shoes that are too tight exert localized pressure on already compressed tissue. This leads to chafing, blistering and pressure necrosis. Equally dangerous are shoes with hard edges or internal seams that press into delicate skin.

Errors during home foot care represent another common cause of complications. Using sharp tools such as nail scissors, clippers, blades or corn razors inevitably causes microtrauma. Even minimal tears in the epidermis are sufficient to breach the epidermal protective barrier. Equally harmful is the use of aggressive callus files or chemical callus removers, which dehydrate the skin and disrupt its natural protective acid mantle, which has a physiological pH of approximately 5.5.

Deformities of the foot skeleton such as hallux valgus, hammer toes or pes plano-valgus alter gait biomechanics. Excessive callosities form extremely rapidly at biomechanical pressure points under the load of the oedema. Pressure ulcers or tissue fluid accumulations can form unnoticed beneath these thick calluses, providing an ideal breeding ground for pathogens.

A 68-year-old female patient from Upper Swabia (Oberschwaben) with secondary lymphedema following surgical removal of a gynaecological tumour attempted to remove hardened callus on her big toe using a rasp. Slipping with the tool caused a small two-millimetre abrasion. Within 36 hours, a rapidly ascending erythema developed accompanied by high fever, requiring inpatient intravenous antibiotic therapy. This case illustrates how sensitively altered tissue reacts to mechanical trauma.

When Medical Evaluation is Required

Due to impaired local immunity within oedematous tissue, bacterial infections can spread within hours. Affected individuals must be thoroughly familiar with the warning signs of an incipient infection and seek immediate medical attention if they appear.

An absolute alarm signal is the onset of spreading, poorly demarcated or flame-like skin erythema that feels locally warm or hot. This is the classic presentation of erysipelas, commonly caused by group A beta-haemolytic streptococci. Erysipelas is frequently accompanied by a rapid rise in body temperature above 38.5 degrees Celsius, chills, headache and pronounced malaise.

Further symptoms requiring immediate medical evaluation include:

  • Increasing, throbbing pain in the foot or lower leg occurring independently of weight-bearing.
  • Sudden, unilateral volume increase in swelling with marked skin tension.
  • Oozing skin areas, blistering or leakage of clear to yellowish tissue fluid.
  • Deeply fissured skin folds between the toes with whitish macerated margins or malodorous deposits.
  • Dark, purple or black skin discoloration indicating tissue necrosis.

If any of these signs are present, immediate action is essential. Delayed treatment of erysipelas can cause permanent damage to remaining lymphatic vessels, irreversibly worsening the lymphedema.

What Podiatric Treatment Can Achieve

Qualified podiatric therapy represents an indispensable component of Complex Decongestive Therapy (CDT). The primary objective of treatment is to correct callosities and nail alterations without causing injury, safeguard the barrier function of the skin and systematically eliminate potential portals of entry for pathogens.

In clinical practice, the use of sharp, cutting instruments such as scalpel blades or skin scissors is strictly avoided in lymphedema patients. Treatment of the skin and nails is performed exclusively using rotary instruments with specialized abrasive burrs. High-precision diamond burrs with extremely fine grit, ceramic burrs and special round burrs with rounded tips are utilized. These instruments are operated at controlled, reduced rotational speeds of approximately 6,000 to 10,000 revolutions per minute. State-of-the-art suction or spray techniques prevent thermal damage to tissue caused by frictional heat.

Debridement of hyperkeratosis is performed gradually and gently. A paper-thin layer of stratum corneum is deliberately left intact to avoid weakening the protective epidermal function. The nail fold is carefully cleaned and cleared of hyperkeratosis using blunt specialized probes. If thickened nails or onychomycosis are present, they are thinned painlessly to reduce pressure on the sensitive nail bed and surrounding nail fold.

Another focus rests on professional skin care. Following mechanical treatment, medical skincare products tailored specifically to the needs of lymphedematous skin are applied. These include water-in-oil (W/O) emulsions containing a moderate urea concentration of 5 to a maximum of 10 percent, alongside lipids that regenerate the epidermal lipid barrier. Treatment typically lasts between 45 and 60 minutes and should be repeated at regular intervals of four to six weeks.

What You Can Do Yourself

Daily home care is decisive in determining whether the skin remains intact and infections can be prevented. Patients should establish a consistent care routine that integrates easily into daily life.

The following steps must be strictly observed during daily self-care:

  1. Daily visual inspection of the feet is essential. Use an unbreakable hand mirror or ask family members for assistance to examine the soles and interdigital spaces for redness, blisters, abrasions or fissures.
  2. Cleansing should be performed with lukewarm water below 37 degrees Celsius. Use exclusively soap-free, pH-skin-neutral synthetic detergents (syndets). Avoid foot baths lasting longer than three minutes, as prolonged soaking macerates the skin and dilates blood vessels, exacerbating swelling.
  3. After washing, the feet must be dried thoroughly yet gently. Pat the skin dry with a soft towel. The interdigital spaces require particularly careful drying, if necessary using a hairdryer set to cool air.
  4. Apply a nourishing, unperfumed emulsion to the dorsum and sole of the foot daily. Consistently omit the interdigital spaces when applying cream, as residual cream binds moisture and promotes fungal infections.
  5. Ensure pressure-free hosiery and footwear. Socks and stockings should not have constricting cuffs. Shoes must offer sufficient volume, be flexible and feature a soft interior finish without irritating seams.

Common Mistakes

In clinical practice, recurring behaviours are frequently observed that significantly increase the risk of tissue damage and infection in lymphedema.

Typical errors in managing the condition include:

  1. Walking barefoot inside the house, in the garden or on the beach poses an immense risk. Even tiny pebbles, insect bites or thorns produce microtraumas that serve as portals of entry for bacteria.
  2. Using pointed or cutting manicure tools such as nail clippers, metal nail scissors or cuticle nippers frequently leads to unintentional injuries around the nail folds.
  3. Independently cutting out ingrown nail corners or manipulating hardened calluses with blades or rasps provokes tissue trauma.
  4. Neglecting the interdigital spaces leads to the development of interdigital mycosis. Fungal maceration of the skin creates microscopic fissures through which streptococci can easily penetrate into the tissue.
  5. Applying hot water bottles or taking hot foot baths for relaxation causes arterial hyperaemia, which increases fluid influx into the tissue and acutely worsens the oedema.

Scientific Evidence and Clinical Context

Medical evidence highlights the paramount importance of injury prevention and skin care in patients with chronic lymphedema. Clinical studies demonstrate that over 70 percent of all recurrent erysipelas episodes in the lower extremities originate from identifiable portals of entry on the foot [1]. Primary causes include interdigital mycosis, traumatic skin lesions and untreated rhagades.

The current S2k guideline "Diagnostics and Therapy of Lymphedema" published by the Association of the Scientific Medical Societies in Germany (AWMF) explicitly emphasizes that decongestive therapy must be complemented by structured, non-traumatic skin care [2]. Avoiding microtrauma is defined therein as essential prophylaxis to prevent infectious complications.

Accompanying podiatric studies indicate that regular, non-invasive debridement of hyperkeratosis using rotary instruments can reduce the incidence of tissue lesions by more than 80 percent [3]. Controlled pressure relief in vulnerable foot regions contributes significantly to maintaining tissue integrity even in advanced stages of the disease.

Furthermore, multicentre analyses on the care of oedema patients demonstrate that an interdisciplinary network model consisting of lymphologists, dermatologists, physiotherapists and podiatrists drastically lowers hospital admission rates for complications [4]. Continuous clinical documentation during medical foot care enables early detection of skin alterations before clinically manifest infections develop.

Compression and Skin Protection in the Oedematous Foot

Medical compression therapy forms the backbone of decongestive treatment. Flat-knit compression stockings, which are utilized in lower extremity lymphedema due to their high working pressure, generate persistent mechanical friction on the epidermis. Shear forces can occur particularly at exposed skeletal structures such as the malleoli, Achilles tendon and dorsum of the foot. Without adequate protective measures, these forces lead to blisters, skin abrasions or pressure-induced ulcerations.

Furthermore, sweat accumulates beneath the dense knit fabric. The resulting trapped moisture softens the stratum corneum of the skin. A macerated horny layer loses its protective function against mechanical strain and microorganisms. Daily skincare must therefore be meticulously timed with the wearing of compression garments. Emollients should preferably be applied in the evening after removing the stockings so that lipids can fully absorb into the skin overnight. Applying cream in the morning immediately prior to donning compression garments is problematic, as fats and oils degrade the elastic material and damage the textile fibres.

Donning and doffing compression garments represents a critical moment in daily routine. Forceful pulling at the toes or heels causes microtrauma to the skin surface. The use of certified donning aids such as donning socks, frame systems or specialized textured gloves noticeably reduces tensile forces on sensitive tissue. In cases of pronounced dorsal foot swelling or deep skin folds, soft padding materials beneath the knit fabric, such as synthetic cast padding or customized foam pads, prevent painful constriction.

Mycosis Prophylaxis and Gentle Correction of Nail Alterations

Dermatophytes and yeasts find ideal growth conditions in the warm, moist microclimate under compression stockings. Interdigital mycosis often progresses insidiously in lymphedema patients, without the typical pruritus. However, fungal infection leads to subtle skin fissures in the interdigital spaces. These microscopic lesions represent the primary portal of entry for streptococci, which trigger feared erysipelas.

In addition to the skin, toenails are frequently affected by alterations. Due to chronic malnourishment of the nail matrix, the nail plates grow back thickened, brittle or deformed. Onychauxis (nail thickening) or onychogryphosis (ram's horn nail) massively increases pressure on the underlying nail bed. When a thickened nail strikes the toe box of the shoe during walking, subungual haematomas or necrosis can develop underneath, which patients often fail to notice promptly due to reduced sensation.

A 74-year-old male patient from the Unterallgäu region suffered recurrent episodes of erysipelas, the cause of which remained unclear for a long time. Only during podiatric assessment was a deep-seated, asymptomatic tinea pedis discovered in the fourth interdigital space, combined with severe onychomycosis of the hallux. Following targeted, non-traumatic treatment of the interdigital spaces and gentle reduction of the affected nail plate, no further infections occurred over a two-year observation period.

Treating mycotic nails demands patience and precision. Rather than aggressive chemical nail polishes or sharp files, water-based medical antifungal preparations combined with fine diamond abrasive technique are utilized in specialist practice. The nail plate is thinned layer by layer without generating heat or injuring the nail fold.

Biomechanical Pressure Relief and Orthosiology

Alterations in gait pattern are a direct consequence of increased volume and weight in the affected extremity. Patients tend to adopt antalgic postures, leading to abnormal loading of the plantar surface. At areas of maximum mechanical load, typically beneath metatarsal heads I and V as well as the calcaneus, the skin responds with hyperkeratosis. Because oedematous tissue is less elastic, this calloused skin readily fissures under pressure, giving rise to painful rhagades extending into the dermis.

To prevent these compensatory responses, podiatric medicine offers custom-fabricated orthoses made of medical-grade silicone. These individual pressure and friction protection devices are contoured precisely to the anatomy of the toes. They gently splay compressed toes, protect bony prominences in hallux valgus or hammer toes, and prevent adjacent phalanges from rubbing against one another inside stockings. The silicone material used is skin-friendly, disinfectable, and retains its elastic properties even under sustained pressure.

Complementing orthosiological management, selecting suitable footwear is of paramount importance. Shoes for lymphedema patients must feature sufficient internal volume to accommodate the foot and compression garment without pinching. Models with removable footbeds are ideal, providing space for cushioned custom orthotics. Hook-and-loop fasteners or flexible lacing systems allow continuous adjustment to fluctuating foot volume throughout the day. Rigid seams in the forefoot area must be strictly avoided to prevent friction spots on the sensitive skin of the dorsal foot.

Frequently asked questions

How does Stemmer's sign differ in lymphedema compared to lipedema?

In lymphedema, the skin fold over the second or third toe cannot be pinched or lifted, or can only be lifted with great difficulty due to the stagnant fluid cushion, which is referred to as a positive Stemmer sign. In lipedema, by contrast, the feet remain free of adipose tissue expansion, allowing the skin fold over the toes to be pinched effortlessly. Stemmer sign is therefore always negative in pure lipedema.

Why should nail scissors not be used in lymphedema of the foot?

The tissue surrounding the toenails is tensely swollen due to the stagnant edema, and the margins of the nail fold often overlap parts of the nail plate. When trimming with nail scissors, there is an extremely high risk of inadvertently cutting into the adjacent tissue. These micro-injuries serve as entry portals for bacteria and can trigger severe tissue infections such as erysipelas.

How often should podiatric treatment take place for lymphedema?

A treatment interval of four to six weeks has proven effective in clinical practice. This cadence allows regrowing hyperkeratosis and nail changes to be managed safely without injury before troublesome pressure points develop. In cases of severe tendency towards hyperkeratosis or hardened skin folds, a shorter interval may temporarily be advisable.

What role does the cryotherapy chamber play in foot swelling?

Whole-body cryotherapy can provide subjective relief from feelings of swelling and heaviness through brief peripheral vasoconstriction followed by anti-inflammatory effects. However, it by no means replaces classic complex decongestive therapy or injury-free skin care. Its use should always be evaluated on an individual basis.

Can podiatric treatment be prescribed for lymphedema?

A medical prescription for podiatry is primarily intended for patients with diabetic foot syndrome or comparable sensory neuropathies. In isolated lymphedema, treatment is generally billed as a self-pay service. However, if secondary complications or neurological deficits are present, a medical prescription may be considered on an individual basis.
