In healthy individuals, the skin on the foot regenerates continuously. Calluses form through physiological pressure loads on the heel cup and the metatarsal heads. When this process occurs harmoniously, a basic home care routine is sufficient. The epithelial tissue reliably protects the deeper dermis from mechanical overload.

However, cellular processes change fundamentally once chronic metabolic or systemic diseases are present. In patients with diabetes mellitus, persistently elevated blood sugar levels lead to diffuse damage to peripheral nerve fibres, known medically as polyneuropathy. In addition, the function of the smallest blood vessels is impaired, a condition termed microangiopathy. Autonomic neuropathy also reduces sweat production in the skin of the foot. The result is an extremely dry, brittle skin barrier that is prone to deep fissures in the heel region.

At the same time, sensory neuropathy abolishes the natural protective sensation. Pressure points, foreign bodies in the shoe, or blisters no longer cause warning pain. Tissue beneath the surface is damaged without the affected person receiving feedback from the body. Hyperkeratosis develops rapidly at sites of increased pressure. Beneath this rigid layer of callus, continuous shear forces often cause haematomas, which can develop unnoticed into deep tissue necrosis.

In inflammatory rheumatic conditions such as rheumatoid arthritis, foot alterations stem from chronic synovitis. Inflammation of the synovial membrane destroys cartilage structures and tendon insertions. This leads to characteristic deformities such as hallux valgus, claw toes or hammer toes, and the collapse of the foot arch. These deformities cause massive shifts in pressure points under the sole of the foot. Areas of skin over bony prominences come under continuous pressure, resulting in painful corns and inflammatory bursitis.

Typical signs

An altered appearance of the feet often indicates that previous care intervals must be shortened. Early signs of a trophic disorder include shiny, thinned skin, reduced hair growth on the dorsal aspect of the toes, and permanently cool toes. When nail deformities such as gryphotic nails, involuted nails, or thickened nail plates develop, the risk of inflammatory processes in the nail fold area increases noticeably.

For patients with diabetes, treatment risk can be classified using standardized criteria defined by the International Working Group on the Diabetic Foot. The presence of peripheral sensory neuropathy is clinically tested using the Semmes-Weinstein monofilament with a test force of 10 grams. If peripheral arterial disease is also present, evidenced by diminished foot pulses in the dorsalis pedis artery and posterior tibial artery as well as a reduced ankle-brachial index, the risk category increases significantly. If deformities of the toes or foot biomechanics are also observed, the tissue is at extreme risk. In such constellations, even minimal hyperkeratosis can lead to visible tissue damage within a few weeks.

In rheumatic feet, pressure pain, erythema over the metatarsal heads, and subluxations of the toe joints predominate. When the cushioning fat pad of the sole migrates anteriorly, the bone heads lie directly beneath the skin. Every step produces direct micromechanical trauma. Rapid recurrence of calluses within a few days of podiatric treatment indicates that biomechanical offloading via footwear or orthotics is not yet sufficient.

Everyday causes

Ill-fitting footwear is among the most common causes of acute and chronic foot problems. Shoes that are too tight, too short, or rigid generate continuous shear forces on the soft tissue. In patients with reduced pain perception, rubbing seams or excessively small shoe sizes often remain unnoticed for years. The material rubs continuously against the toe joints or heel, massively accelerating callus formation.

Another everyday issue is trimming toenails incorrectly. If the nail edges are rounded off deep into the lateral nail fold, fine spicules or sharp edges often remain. As the nail grows back, the nail plate embeds into the surrounding soft tissue. An ingrown toenail develops, accompanied by painful inflammation, granulation tissue, and bacterial colonization.

The use of aggressive tools during home foot care represents another significant risk. Callus planes, blades, or electric grinders often remove excessive tissue. In a protective response, the skin produces even thicker layers of callus within a short time. Furthermore, microscopic cuts occur. These injuries form an ideal portal of entry for pathogens such as Staphylococcus aureus or fungal organisms.

Moisture caused by unsuitable hosiery made from synthetic fibres promotes skin maceration. The softened skin in the interdigital spaces loses its natural barrier function against dermatophytes. Fungal infections occur, which can unnoticedly damage deeper skin layers and result in secondary infections.

When medical evaluation is necessary

Certain foot symptoms require immediate consultation with a specialist in diabetology, dermatology, vascular surgery, or orthopaedics. In high-risk patients, delay can result in severe tissue loss or systemic infections. This applies particularly to the onset of acute skin defects, ulcerations, or open wounds, regardless of their initial size.

Similarly, local signs of inflammation such as erythema, hyperthermia, swelling, or purulent secretion necessitate immediate medical attention. Blackish-blue discolouration of toes or tissue areas indicates necrosis and represents an emergency. Sudden, severe rest pain in the foot or leg, particularly when elevating the limb, points to an acute deterioration in arterial blood flow.

Urgent action is also required in the presence of systemic signs of infection such as fever, chills, or a rapid, unexplained rise in blood glucose levels. Once an ulcer is present, purely conservative podiatric care must not be carried out without a medical treatment plan. Furthermore, if acute Charcot neuroarthropathy is suspected, presenting as a painless, red, and warm swelling of the foot in diabetic patients without prior trauma, the foot must be offloaded immediately and evaluated radiologically.

What podiatric treatment can achieve

Podiatric therapy differs fundamentally from cosmetic foot care. As a recognized healthcare profession, podiatry operates on the basis of precise anatomical knowledge, strict hygiene regulations, and specialized instrumentation. During an initial consultation at the practice, a comprehensive medical history is taken.   • Targeted skin care: Apply a moisturizing cream to your feet daily. Products containing 5 to 10 percent urea bind moisture in the tissue and keep the skin supple. Avoid applying cream between the toes to prevent a moist environment from forming.
  • Mindful selection of hosiery and footwear: Wear socks made from natural materials such as cotton or merino wool without constricting seams. The waistband should not cut in. Feel the inside of your shoes with your hand before putting them on to detect foreign objects or folded insoles.

Common mistakes

The acid does not distinguish between hyperkeratotic and healthy skin. Improper nail trimming with sharp paper scissors or clippers also frequently leads to injuries of the nail fold.
