What lies behind it

The anatomical structure at the center of plantar heel pain is the plantar fascia. This is a strong, aponeurotic band of connective tissue that originates on the underside of the calcaneus and fans out towards the metatarsophalangeal joints of all five toes. This structure maintains tension in the longitudinal arch of the foot and absorbs significant mechanical forces with every step. This bony prominence on the calcaneus is referred to as a plantar calcaneal spur.

The bone spur itself is visible on X-rays in many individuals, yet it by no means always causes pain. A heel spur is frequently detected as an incidental finding in asymptomatic individuals. The actual pain originates primarily in the irritated, thickened, and microtraumatised tendinous tissue and the surrounding periosteum. The thickness of the plantar fascia increases significantly during pathological processes. While healthy tissue measures approximately two to three millimetres in thickness on ultrasound, the affected aponeurosis often shows a thickness of five millimetres or more. This thickening usually correlates directly with the severity of symptoms.

Typical signs

The cardinal symptom of the condition is pronounced start-up pain. This typically occurs when weight is first placed on the foot following a period of rest. Getting up in the morning or standing up after sitting for an extended period is experienced as particularly painful. During the resting phase, the shortened fascia contracts. When the affected person suddenly steps down, the scar and inflamed tissue are abruptly stretched, triggering characteristic sharp pain impulses. After a few minutes of gentle movement, the pain noticeably subsides as the tissue adapts to the stretching stimuli and blood circulation improves.

However, under continued weight-bearing throughout the day, the pain returns. It often transforms into a dull, burning, or aching persistent pain. Pressure applied to the medial process of the calcaneal tuberosity, the inner underside of the heel, reveals a sharply localized point of tenderness. Walking on hard surfaces without shock-absorbing footwear drastically aggravates the symptoms.

A differential diagnosis is essential, as various other conditions can cause similar symptoms in the heel area. Differential diagnoses include:

  • Compression of the posterior tibial nerve at the medial ankle, known as tarsal tunnel syndrome.
  • Baxter neuropathy, in which the first branch of the lateral plantar nerve is entrapped.
  • Heel fat pad atrophy, in which the natural fat cushion beneath the calcaneus diminishes.
  • Calcaneal stress fractures, which usually arise from sudden, excessive increases in physical loading.
  • Spondyloarthritides such as ankylosing spondylitis or psoriatic arthritis, which manifest as enthesopathies.
  • Achilles tendon pathologies, which have their focal point of pain primarily at the posterior aspect of the heel.

Causes in everyday life

The causes of plantar fasciopathy are multifaceted and rarely result from a single trigger. In most cases, an individual anatomical predisposition coincides with unfavourable external loading factors. One of the most common internal causes is faulty foot mechanics. A pes plano-valgus or splay foot alters the loading angle of the longitudinal arch. As a result, the plantar fascia is subjected to continuous, excessive tensile stress. Similarly, a pronounced pes cavus, with its rigid structure and lack of shock absorption, represents a significant risk.

A major biomechanical factor is tightness in the posterior muscle chain. Shortened calf muscles, particularly the gastrocnemius and soleus muscles, restrict dorsiflexion in the talocrural joint. During the roll-over phase of gait, the foot must compensate for this lack of mobility, leading to increased strain on the plantar aponeurosis. The gait pattern changes, and the load on the plantar fascia increases exponentially.

External factors exacerbate these structural issues. Frequently wearing flat, insufficiently cushioned footwear without arch support places extreme stress on the tissue. Likewise, an abrupt switch from high-heeled shoes to completely flat footwear can overload the aponeurosis. Prolonged standing or walking on hard industrial floors, as well as unconditioned running on asphalt, subjects the connective tissue to immense strain. A sedentary lifestyle combined with sudden increases in intensity during sports involving frequent jumping and stopping movements provokes microtraumata. Furthermore, increased body weight acts as a constant risk factor, as every additional kilogram multiplies vertical ground reaction forces with every step.

When medical evaluation is necessary

Not all heel pain can be managed purely through podiatric or conservative home care. Certain symptoms require prompt specialist medical evaluation by orthopaedic surgeons or rheumatologists. Persistent pain at rest or during the night is a clear red flag. If the heel continues to throb or burn for hours even when completely offloaded, an inflammatory process in deeper structures or a neurological cause must be investigated.

Likewise, visible erythema, pronounced swelling, or palpable localized warmth across the heel area require rapid medical diagnosis. These signs may indicate a bacterial infection, acute subcalcaneal bursitis, or a systemic rheumatic disease. Numbness, tingling, or radiating electric-like pain extending into the toes or lower leg suggest neural involvement. A sudden popping sound followed by an inability to bear weight points to a partial or complete rupture of the aponeurosis. In patients with pre-existing diabetes mellitus, peripheral arterial disease, or immunosuppressive conditions, any heel symptoms must be monitored by a physician at an early stage.

Grasp the toes of the affected foot with your hand and pull them upwards towards the shin in a controlled manner until you feel a distinct tension in the sole of the foot. With your other hand, hold the calcaneal origin and gently massage along the aponeurosis using your thumb. Hold the stretch for 30 seconds each time and repeat this process three consecutive times. Ideally, this exercise should be performed immediately before taking the first steps in the morning to mitigate classic start-up pain.

To stretch the calf muscles, assume a lunge position facing a wall. The rear leg is the one being stretched. Keep the heel of the rear leg firmly anchored to the floor, with the toes pointing straight ahead. Slowly bend the front knee forward until an intense stretch is felt in the calf of the rear leg. To target the deeper soleus muscle, slightly bend the rear knee while maintaining heel contact with the floor. Both variations should be held two to three times daily for 45 seconds each.

In addition, progressive resistance training of the calf muscles has proven effective. Stand with the forefeet on a stair step, letting the heels extend freely off the edge. Push up onto the toes over a count of three seconds. Hold the peak position for two seconds, then lower the heels below the level of the step in a controlled manner over four seconds. To enhance the load on the plantar fascia, a rolled towel can be placed under the toes. Perform this exercise every other day for three sets of 10 to 12 repetitions.

Common errors

Clinical practice frequently reveals typical mistakes that delay healing or aggravate symptoms. A very common error is complete rest. Affected individuals avoid all load-bearing and immobilise the foot completely. This causes collagenous tissue to lose elasticity, the aponeurotic structure to shorten further, and muscles to atrophy. As soon as weight-bearing is resumed, the pain reaction is even more severe. Appropriate, pain-free movement is essential for the alignment of new collagen fibres.

The opposite extreme is continuing high-intensity sports despite severe pain. Continuing marathon training or wearing unsuitable footwear during indoor sports with acute plantar fasciitis risks irreversible tissue damage. Improper use of fascia rollers also carries risks. Excessive, painful rolling directly over the inflamed calcanean tuberosity leads to additional tissue contusion and exacerbates inflammation. Massage should be gentle and restricted strictly to the soft muscle and tendon tissue of the calf and the mid-plantar region.

Equally problematic is the uncritical purchase of cheap, soft gel heel pads from retail stores. While these cushions provide short-term comfort, they alter shoe fit and offer no structured biomechanical correction for the longitudinal arch. They do not address the underlying cause of the altered tensile angle. Finally, stretching for overly short durations of merely five to ten seconds fails to produce the desired adaptive changes in tendinous and muscular tissue.

Medical research in recent years has fundamentally transformed the understanding of plantar fasciopathy.
