What lies behind the condition
The medical term hallux valgus refers to a structural deformity affecting the first ray of the foot. The first ray consists of the first metatarsal bone (os metatarsale I) and the two phalanges of the great toe. In a healthy state, these bones align in a virtually straight line. In hallux valgus, however, the first metatarsal bone deviates medially towards the inner margin of the foot. Medical professionals refer to this as metatarsus primus varus. Simultaneously, the phalanx of the great toe tilts laterally towards the second toe, which is defined as a valgus deviation.
Due to this opposing displacement, the first metatarsophalangeal joint is forced out of its natural axis. The head of the metatarsal bone protrudes noticeably on the inner side of the foot. This creates the characteristic prominence commonly referred to as a bunion or pseudoexostosis. This bump is primarily not a true bony overgrowth, but rather the internally displaced metatarsal head lying unprotected directly beneath the skin.
As the condition progresses, the biomechanical forces exerted by the surrounding tendons are altered. The abductor hallucis muscle, which is responsible for abducting the great toe, loses its mechanical leverage and slips inferiorly. At the same time, the adductor hallucis muscle pulls the toe further outward. The tendons of the long toe flexors and extensors now act like the string of a drawn bow: they automatically exacerbate the deformity with every step. In addition, the two small sesamoid bones (ossa sesamoidea), which serve as a sliding mechanism beneath the metatarsal head, shift out of their anatomical groove.
Continuous mechanical friction between the protruding bunion and the interior of the shoe rapidly leads to chronic irritation of the skin and underlying soft tissue. A bursa (bursa subcutanea metatarsophalangea) frequently forms over the joint. This bursa serves as a natural buffer against pressure. Under sustained mechanical stress, however, it becomes regularly inflamed, swells painfully, and can even develop a purulent infection if pathogens enter.
Typical signs
The most noticeable symptom of hallux valgus is the visually apparent misalignment of the great toe, accompanied by the lateral protrusion along the inner foot margin. However, this deformity is associated with numerous secondary features that extend far beyond the appearance of the first toe. Due to the deviation of the first metatarsal bone, the forefoot loses the structural support of its transverse arch. A pronounced splayfoot develops, forcing the central metatarsal heads with increased pressure against the ground.
Affected individuals frequently notice the following symptoms on their feet:
- Pressure pain, redness, and localized hyperthermia directly over the bunion prominence after prolonged walking or standing
- Pronounced callus formation (hyperkeratosis) or painful corns at the pressure points of the bunion and beneath the heads of the second and third metatarsals
- Displacement of adjacent toes, which often adapt by assuming a hammer toe or claw toe position
- Transfer metatarsalgie, a burning pain beneath the central forefoot area caused by improper load distribution during gait roll-off
- Progressive restriction of movement and stiffness in the first metatarsophalangeal joint due to advanced joint degeneration
Careful differential diagnosis to rule out other conditions of the first metatarsophalangeal joint is essential. In hallux rigidus, for example, there is painful osteoarthritis of the first metatarsophalangeal joint without significant lateral deviation of the toe. In this case, joint mobility is severely restricted, particularly during upward movement (dorsiflexion), and bony spurs often form on the upper surface. Gout (gouty arthritis), on the other hand, typically presents as a sudden, extremely painful flare-up. The joint becomes intensely red, hot, swollen, and exquisitely sensitive to even the slightest touch. Rheumatoid arthritis or psoriatic arthritis can also affect the metatarsophalangeal joint, but usually exhibits a symmetrical pattern of involvement alongside specific inflammatory markers in the blood.
Everyday causes
The development of hallux valgus can rarely be attributed to a single cause. In most cases, it stems from an interplay of genetic predisposition, individual foot mechanics, and external lifestyle factors. A congenital weakness of the connective tissue plays a central role. When ligaments and tendons are unable to stabilize the foot arch adequately, the longitudinal and transverse arches collapse under body weight. The foot widens, causing the first metatarsal bone to shift medially.
In individuals with a corresponding predisposition, footwear acts as a significant accelerator of the deformity. High-heeled shoes shift the entire body weight forward onto the forefoot, multiplying the load placed on the metatarsal heads. If the shoe additionally tapers towards the front, the toes are compressed within a narrow space. The great toe is continuously forced into a valgus position. Over the course of years, joint capsules, ligaments, and muscles adapt to this malposition and permanently shorten.
Another everyday factor involves abnormal mechanical loading during walking, which is exacerbated by an uncoordinated gait or obesity. A lack of movement in the foot musculature also contributes to deterioration. Anyone who deprives their feet of variety and walks exclusively on hard, level surfaces in rigid footwear weakens the intrinsic foot muscles. Yet these small muscles are essential for maintaining the correct alignment of the forefoot bones.
Improper care indirectly compounds the problem. If callus layers at pressure points are removed improperly or with excessively sharp tools, the skin responds by regenerating even faster. Incorrect trimming of the toenails, such as rounding off the corners too deeply, can also lead to ingrown nails. Pain caused by ingrown nails further alters gait mechanics, as affected individuals attempt to protect the painful toe while walking, thereby placing abnormal stress on the rest of the foot.
When medical evaluation is necessary
Hallux valgus is a progressive condition. Although podiatric measures can significantly improve wearing comfort and alleviate pain, there are clear warning signs that require evaluation by an orthopaedic surgeon or medical specialist. Timely medical assessment protects against permanent secondary damage to the skeletal system and prevents chronic inflammatory processes.
Consult a physician if you notice any of the following changes:
- Pain at rest or throbbing pain occurring independently of weight-bearing or footwear, especially at night
- Clear signs of infection, such as widespread redness, pronounced warmth, increasing swelling, or purulent discharge over the bunion prominence
- Numbness, tingling, or shooting pains in the great toe indicative of nerve compression
- Rapid progression of the deformity within a few months, accompanied by a severe restriction of walking distance to under 500 metres
- Presence of underlying conditions such as diabetes mellitus or peripheral arterial disease, where even minor pressure points can lead to slow-healing wounds
An orthopaedic specialist will conduct a clinical examination and order weight-bearing foot radiographs. These X-rays allow for the precise measurement of the hallux valgus angle and the intermetatarsal angle. These measurements provide the decision-making basis for determining whether conservative therapy remains sufficient or whether surgical intervention for realigning the bones should be considered.
What podiatric treatment can achieve
Podiatry plays a central role in the conservative management of hallux valgus. The primary goal of treatment is to relieve mechanical pressure from the painful bunion, eliminate secondary skin damage, and restore the functional load-bearing capacity of the foot in daily life. A qualified podiatric assessment evaluates the exact condition of the skin, the distribution of hyperkeratosis, and the joint mobility of the forefoot.
An essential component of regular treatment is the professional removal of pressure-induced hyperkeratosis. Using scalpel techniques and fine rotary instruments, the podiatrist removes excessive callus layers painlessly and precisely. This immediately reduces tissue pressure over the irritated metatarsophalangeal joint. If constant pressure has led to the formation of corns between the toes or on adjacent hammer toes, these are gently enucleated and expertly treated.
A highly effective tool in modern podiatry is the fabrication of custom silicone orthoses. In contrast to prefabricated off-the-shelf products, these protective appliances against pressure and friction are moulded directly on the patient's foot using medical-grade two-component silicones. Depending on the indication, different firmness levels (Shore hardness) are utilized. Soft silicone is ideal for purely relieving pressure on the sensitive bunion region. Medium to firm material is used as a toe separator between the first and second toes to gently correct the deviation of the great toe and prevent toes from overlapping. Treatment intervals depend on individual clinical findings and the rate of callus regeneration. Generally, intervals of four to six weeks have proven effective. During these appointments, skin care is performed, and the fit and functionality of the silicone orthosis are reviewed and adjusted as necessary.
What you can do yourself
Successful relief from hallux valgus discomfort requires your active participation in daily life. Even the most effective podiatric measures will fail to achieve their full effect if the foot remains exposed to restrictive footwear or damaging mechanical stress outside the clinic. With structured self-care, you can slow down the progression of the deformity.
Review your footwear critically. A foot-appropriate shoe provides sufficient space in the forefoot region so that the toes are not compressed laterally. When buying shoes, look for a wide toe box and soft, seamless upper leather over the bunion area. Heel height should not exceed two centimetres in daily life. Flat shoes allow for even weight distribution across the entire sole and provide long-term relief to the metatarsal heads.
Incorporate targeted foot exercises into your daily routine. Regular exercise strengthens the intrinsic foot muscles and promotes toe mobility. A proven exercise is the so-called "short foot" exercise according to Janda. In this exercise, you attempt to pull the ball of the forefoot and the heel towards each other on the floor without curling the toes. This noticeably lifts the longitudinal arch. Actively spreading the toes is equally effective: try to splay the great toe medially while extending the small toes laterally. Ten minutes of daily exercise over several months yields measurable success in dynamic foot stabilization.
Ensure proper care for your custom silicone orthosis. Wash the silicone piece daily using mild soap and lukewarm water. Allow it to air dry, then lightly dust it with body powder or talcum powder. This maintains the elasticity of the material, prevents stickiness, and ensures a comfortable feel against the skin.
Common mistakes
In podiatric practice, we frequently observe typical behaviors that unintentionally worsen the course of the condition or lead to additional complications. A widespread misconception is purchasing universal off-the-shelf silicone toe spreaders from drugstores. These standard products are rarely adapted to individual foot anatomy. They are often excessively thick, forcing the great toe outward and creating massive pressure on the second and third toes. This can lead to painful corns or joint dislocations in those areas.
Another mistake is relying entirely on so-called hallux valgus night splints with the promise of complete alignment correction. A fixed bony deformity in adults cannot be reversed by a purely passive splint. Although night splints can stretch soft tissues and the joint capsule while easing pain, they do not alter the changed bony axes. Relying solely on a splint to resolve the issue often leads to neglecting necessary changes in footwear and active muscle training.
Attempting to remove corns or calluses on one's own using razor blades, corn planes, or aggressive corn plasters from the pharmacy is highly dangerous. Corn plasters usually contain salicylic acid. If applied incorrectly, this acid burns away the healthy tissue surrounding the pressure point, resulting in deep wounds that can become infected. Manipulating sharp blades independently also carries a severe risk of injury, as slipping blades can easily cause deep cuts.
Finally, persistently wearing shoes that are too tight for fashion reasons is a primary driver of rapid disease progression. Continuing to wear narrow, pointed shoes despite existing pain knowingly leads to the consolidation of the deformity and exceeds the load tolerance of the joint.
Current scientific evidence
Medical research deals extensively with the efficacy of conservative and surgical therapies for hallux valgus. There is widespread consensus in scientific literature that purely conservative measures cannot permanently correct a fixed bony deformity anatomically. Nevertheless, clinical studies demonstrate the high relevance of conservative interventions for symptom control and patient quality of life [1].
Systematic reviews demonstrate that combining custom-fitted pressure relief appliances, orthotic insoles, and targeted physiotherapy can significantly reduce pain in mild to moderate forms of hallux valgus [2]. Research findings also highlight that custom-made silicone orthoses achieve significantly higher patient satisfaction and better pressure distribution over the bunion compared to ill-fitting off-the-shelf products. The biomechanical unloading of the second and third metatarsal heads leads to a measurable reduction in secondary metatarsalgia [3].
Regarding surgical procedures, long-term observational studies show that while surgical correction (such as Scarf or Chevron osteotomy) provides excellent radiological correction results, the postoperative recovery phase is lengthy [4]. Surgical risks such as wound healing complications, recurrence, or residual discomfort require strict indications. Scientific guidelines therefore consistently recommend considering surgery only after exhausting all conservative treatment options, unless irreversible joint damage or severe pain at rest is present.

Which shoes are best suited for hallux valgus?

Heels should be flat to prevent excessive loading on the forefoot.
