What lies behind it
Human skin on the foot performs a complex protective function under extreme mechanical stress. Its outermost layer, the stratum corneum, consists of dead corneocytes embedded in a matrix of epidermal lipids. This composite system prevents the penetration of pathogens and chemical substances while protecting the tissue from dehydration. If this barrier becomes imbalanced, inflammatory skin reactions occur, which are clinically referred to as eczema.
Eczema is a non-infectious inflammation of the skin that can be triggered by various internal and external factors. In allergic contact dermatitis, the immune system reacts hypersensitively to specific foreign substances that come into contact with the skin. This represents a type IV delayed hypersensitivity reaction. After initial contact with an allergen, days to weeks pass during which T lymphocytes become sensitised to this substance. Upon re-exposure, these immune cells release inflammatory mediators, leading to erythema, swelling, and vesicle formation.
In contrast, tinea pedis is the classic fungal foot infection. In most cases, it is caused by dermatophytes, particularly the pathogens Trichophyton rubrum or Trichophyton interdigitale. These filamentous fungi feed on the keratin of the human stratum corneum. They thrive especially well in a warm, moist environment, such as that created inside closed footwear. The enzyme secretion of dermatophytes triggers an inflammatory response in the host tissue that can closely resemble eczema clinically.
Additionally, dyshidrotic eczema, also known as pompholyx, exists. This manifestation presents with deep-seated, tense vesicles on the margins of the toes or the soles of the feet. It can occur both as a result of a contact allergy and as an id reaction to an existing fungal infection elsewhere on the body. Precise differentiation therefore requires a sound physiological understanding.
Typical signs
Differentiating between contact dermatitis and tinea pedis is primarily achieved through the distribution pattern and the specific appearance of the skin lesions. A thorough inspection of the entire foot provides valuable clues regarding the underlying etiology.
Allergic contact dermatitis presents predominantly in areas that had direct contact with the trigger. If the reaction involves ingredients of the upper leather or waterproofing agents, the dorsum of the foot is typically erythematous. The interdigital spaces are conspicuously spared, as the material rarely rests flat against them. The skin shows ill-defined erythema, papules, and fine vesicles that form crusts after rupturing. Patients usually describe the pruritus as an unbearably burning sensation.
Tinea pedis, on the other hand, manifests in three main forms. The interdigital form almost always begins in the web space between the fourth and fifth toes. The skin macerates, turns whitish and soft, desquamates, and develops painful fissures, known as rhagades. The squamous-hyperkeratotic form, also called the moccasin type, spreads across the plantar surface and the heel margins. It presents with fine, flour-like scaling on dry, mildly erythematous skin and is often mistaken for extremely dry skin.
Vesicular-dyshidrotic tinea pedis presents with pruritic, tense vesicles along the arch of the foot. In contrast to contact allergies, lesions in fungal infections are often asymmetrically distributed. A bilateral, symmetrical manifestation on both feet suggests a systemic or allergic cause, whereas mycoses frequently start on one foot and only affect the contralateral side after a delay.
Triggers in everyday life
Everyday life offers numerous triggers for barrier disruptions and inflammation on the feet. Perspiration, mechanical friction, and contact with synthetic chemicals form a complex web of risk factors.
Modern footwear utilizes countless chemical compounds. Tanning agents such as chromium VI salts in leather, vulcanisation accelerators such as thiurams or carbamates in rubber soles, and adhesives based on para-tertiary-butylphenol formaldehyde resin possess high allergic potential. Dyes in socks or finishing agents such as biocides can also trigger contact dermatitis in sensitive individuals. If the foot remains inside impermeable footwear for hours, sweat dissolves the allergen molecules from the material and transports them directly into the softened skin.
A typical case example illustrates this problem. A 44-year-old field representative wore leather safety shoes of protection class S3 for over ten hours daily. After a few months, pruritic, desquamating erythema developed across the entire dorsum of both feet. The general practitioner treated for athlete's foot for months without improvement. Only after patch testing was performed did it turn out that an allergy to potassium dichromate was present. Switching to chromium-free tanned footwear led to the resolution of symptoms within a few weeks.
Alongside allergic triggers, the microclimate inside the shoe plays a decisive role in fungal infections. Chronic moisture causes the stratum corneum to swell. Intercellular lipids are washed out, facilitating the penetration of dermatophytes into deeper tissue layers. Inadequate drying after showering or exercise also promotes rapid fungal growth.

Not every skin alteration on the foot can be managed independently or solely through routine care. Certain symptoms strictly require immediate diagnosis and treatment by a specialist in dermatology.
An urgent warning sign is the appearance of signs of a bacterial superinfection. If the skin is severely painful, exhibits local hyperthermia, forms yellowish crusts, or secretes purulent exudate, bacteria such as Staphylococcus aureus have overgrown the damaged barrier. - Change socks daily and wash them at a minimum of 60 degrees Celsius. Preferably use socks made of undyed cotton or silver fibres without synthetic blends.
- Rotate your shoes daily. Footwear requires at least 24 to 48 hours to dry completely inside and break down perspiration residues.
- Maintain foot skin with emulsifier-free, lipid-rich creams or foam systems. Active ingredients such as urea in a concentration of 5 to 10 percent bind moisture, while ceramides strengthen the barrier.
- Avoid harsh foot baths with curd soap or alcohol rubs during acute inflammation. These strip valuable lipids from the skin and exacerbate pruritus.
Common mistakes
In practice, typical behavioural patterns frequently emerge that unintentionally worsen the skin's condition. The most frequent mistake is unguided self-assessment and self-treatment with over-the-counter ointments from the pharmacy.
If a fungal infection is mistakenly treated with an over-the-counter cream containing hydrocortisone, relief from pruritus initially occurs. However, the hydrocortisone suppresses the local immune response of the skin. Dermatophytes can spread unhindered into deeper layers of the epidermis. This creates the presentation of tinea incognito, a masked fungal infection that is clinically difficult to diagnose and usually results in extensive tissue damage.
An equally severe mistake is applying highly concentrated essential oils such as tea tree oil to open or inflamed tissue. Tea tree oil contains numerous terpenes, which themselves possess high allergic potential. What was intended as a natural remedy against athlete's foot not infrequently triggers an additional allergic contact dermatitis, complicating the clinical picture significantly.
Mechanical scratching of vesicles or tearing off skin scales also severely impairs the healing process. Through microtrauma, bacteria from the normal skin flora enter the subcutaneous tissue. The consequence is painful inflammation that delays the healing process by weeks.
Finally, footwear hygiene is frequently neglected. Treating only the skin while continuing to wear the same infected or allergen-contaminated shoes without disinfection or insole replacement leads to continuous relapses. Epidemiological surveys show that tinea pedis is among the most common infectious diseases in Central Europe. Approximately 20 percent of the adult population exhibits signs of a fungal foot infection [1]. In diabetic patients, the prevalence is significantly higher.
Studies on shoe contact allergy demonstrate that approximately 3 to 11 percent of all patients with contact eczema react allergically to components of footwear [2]. The most frequently identified contact allergen is potassium dichromate, followed by resins and vulcanisation accelerators [3]. These studies highlight that differentiation without instrumental or laboratory diagnostics is challenging even for experienced clinicians, as the morphology of the lesions can overlap.
Scientific guidelines for diagnosing tinea pedis emphasize the necessity of microscopic and cultural pathogen identification before initiating systemic antifungal therapy [4]. The sensitivity of a simple wet mount exceeds 70 percent when performed by experienced practitioners, while PCR diagnostics allows for even higher accuracy and rapid pathogen identification [5]. Differentiation from related clinical conditions
Not every desquamative or vesicle-forming skin lesion on the lower extremities can be unequivocally attributed to a mycosis or a contact allergy. In dermatological practice, palmoplantar pustular psoriasis and atopic foot eczema represent common differential diagnoses. Palmoplantar psoriasis presents with sterile, deep-seated pustules on sharply demarcated, markedly erythematous plaques on the soles of the feet. In contrast to infectious tinea pedis, no pathogens can be cultured from these pustules, as this represents an abacterial autoimmune inflammatory response.
Atopic foot eczema occurs primarily in individuals with a genetic predisposition to atopic dermatitis or allergic rhinitis. It typically manifests as a dry, intensely pruritic eczema on the toe pads, clinically referred to as pulpitis sicca. The skin desquamates in fine lamellae and forms painful fissures at pressure points. A rare but easily confused bacterial infection is pitted keratolysis (keratolysis sulcata). Caused by Corynebacterium species, this keratolysis produces pitted, crater-like defects in the macerated stratum corneum of the sole and is accompanied by a distinctive odour.
Precise differentiation of these clinical pictures is therapeutically crucial. While topical corticosteroids provide rapid relief in atopic eczema and psoriasis, pitted keratolysis requires the targeted use of topical antibiotics such as clindamycin or erythromycin. Pharmacological treatment requires strict selection of active substances based on indication. In confirmed tinea pedis, broad-spectrum antifungals form the mainstay of topical therapy. Substances from the allylamine group, such as terbinafine, exert a primarily fungicidal effect on dermatophytes by inhibiting squalene epoxidase, thereby blocking ergosterol synthesis in the fungal cell wall. A treatment duration of at least two to four weeks is necessary to completely clear deeper layers of the stratum corneum. Alternatively, azole antifungals such as clotrimazole or miconazole are utilized, covering a broad pathogen spectrum while acting predominantly fungistatically.
If confirmed allergic contact dermatitis is present, interrupting the inflammatory cascade is the primary objective. Short-term application of class II or class III glucocorticoids such as mometasone furoate or hydrocortisone butyrate relieves erythema and pruritus within a few days. In chronic cases or where steroids are contraindicated, topical calcineurin inhibitors such as tacrolimus offer a steroid-free alternative. Concurrently, synthetic tanning agents such as phenolsulfonic acid condensates play an important role. They act as astringents, strengthen the protein structures of the damaged skin barrier, and significantly reduce weeping in exudative eczema lesions.
In severe, refractory cases, topical treatment alone is usually insufficient. Systemic antifungals such as itraconazole or terbinafine tablets must be administered for several weeks if the moccasin type of fungal foot infection extensively involves the entire plantar surface. Such systemic therapies require regular laboratory monitoring of liver function parameters by the treating physician.

To prevent skin maceration, the use of functional socks containing a high proportion of merino wool or specialized synthetic fibres such as polypropylene is recommended. These capillary fibres wick moisture away from the foot into the inner lining of the shoe rather than retaining it in the fabric like pure cotton.
