At the same time, the motor component causes an imbalance in the foot muscles. The short foot muscles atrophy, leading to deformities such as hammer toes or claw toes. As a result, body weight shifts during walking onto small bony prominences beneath the foot that are not designed for this load.
Additionally, autonomic neuropathy impairs sweat gland regulation. The skin on the foot releases hardly any moisture, becoming extremely dry, brittle and losing its elasticity. Deep skin cracks develop, known as rhagades, which serve as an entry point for bacteria. If the microcirculation or major arteries are simultaneously narrowed due to arteriosclerosis, insufficient oxygenated blood reaches the tissue. Small wounds no longer heal because the body cannot transport the required nutrients and immune cells to the site of injury.
Typical signs
The initial changes to the feet often announce themselves subtly. In the early stages, affected individuals report a tingling sensation reminiscent of crawling ants or burning dysaesthesia that increases particularly in the evening hours. A striking phenomenon is the feeling of walking on cotton wool or a thick layer of rubber. Normal perception of the ground beneath is lost.
Visible signs of risk include unusual calluses at points of high pressure. Hemorrhages or cavities often form unnoticed beneath these hyperkeratoses, representing a precursor to malum perforans, a deep diabetic ulcer. Changes in skin colour also deserve attention. Pale, cool skin points to circulatory disorders, whereas a reddish blue discolouration or localized heat may indicate inflammatory processes or bone remodeling.
These signs must be distinguished from purely venous complaints, where swollen ankles and congestion-related skin changes are usually predominant. Fungal infections of the skin and nails also occur more frequently in people with diabetes. Athlete's foot manifests as scaling and maceration between the toes, whereas nail fungus leads to thickened, crumbly and yellowish discoloured nail plates. Both infections significantly weaken the barrier function of the skin.
Everyday causes
Injuries in the home environment rarely result from major accidents. Most often, careless behaviours or unsuitable everyday items cause damage. Improper footwear ranks as the leading trigger. Many affected individuals purchase shoes that are too small or too narrow because reduced sensation causes the foot to falsely report that the shoe fits well. In reality, the upper material squeezes the toes together, generating blisters or friction on the toe joints.
Mistakes in nail and skin care represent another risk. Using sharp metal instruments such as nail scissors, clippers, blades or callus razors quickly leads to cuts in the sensitive nail folds. The use of callus rasps or pumice stones also frequently creates microscopic tears through which germs penetrate. Over-the-counter corn plasters or tinctures contain salicylic acid. This acid does not distinguish between dead callus and healthy skin. In diabetic neuropathy, it regularly leads to extensive chemical burns of the tissue.
Thermal damage frequently occurs due to foot baths that are too hot. Anyone who can no longer check the water temperature with their foot due to a lack of thermal sensation and foregoes a thermometer quickly suffers second-degree burns. Similarly, walking barefoot in the garden or on carpeted floors represents a source of danger. Pins hidden in carpet fibres, small glass splinters or sharp wooden edges penetrate the plantar soft tissue layer effortlessly.
When medical evaluation is necessary
A diabetic foot requires a strict division of responsibilities. As soon as skin integrity is breached, home care ends. Every wound, blister or cutaneous haemorrhage, no matter how small, must be presented immediately to a general practitioner, diabetologist or specialized foot clinic. Warning signs of an acute infection include:
- Increasing redness spreading around a site.
- Localized warming of the skin compared to the healthy foot.
- Swelling on the dorsum of the foot or around the ankles.
- Discharge of wound fluid, pus or a noticeable unpleasant odour.
- Sudden skin discolouration turning dark red, purple or black.

The musculoskeletal system, vascular status and sensitivity are checked in a structured manner. - Examine the sole of the foot and the heel. If you cannot bend down far enough, use an unbreakable hand mirror with a telescopic handle or place the mirror on the floor.
- Gently stroke the entire sole of the foot with the flat of your hand to feel for bumps, rough areas or embedded foreign bodies.
- Feel the complete inside of each shoe with your hand before putting it on to check for small stones, grains of sand or folded insoles.
Feet should be cleaned daily with lukewarm water. Always check the water temperature with a bath thermometer or your forearm. The bath must last no longer than three minutes, as the skin will otherwise soften and lose its protective barrier. Use mild, soap-free washing lotions. After washing, the feet must be dried thoroughly. Take particular care to gently pat dry the spaces between the toes without rubbing.
Special creams or foam creams with a urea content of 5 to 10 percent are suitable for subsequent care. Urea binds moisture in the stratum corneum and keeps the skin supple. Apply the care product thinly to the dorsum and sole of the foot. The spaces between the toes must be left out, as remaining cream residues there create a moist environment that favours the development of fungal infections.
Footwear must offer sufficient space for the toes. When buying socks, choose soft, natural materials such as cotton or merino wool without pressing seams in the toe area. Avoid socks with tight elastic bands, as these can constrict venous return. Never walk around the house in socks or barefoot: always wear sturdy slippers with a closed heel and non-slip sole.
Common mistakes
In practice, recurring behavioral patterns that lead to complications are seen time and again. Awareness of these mistakes helps patients review their own habits critically.
The most common behavioural errors include:
- Walking barefoot on warm sand, tiles or carpets despite reduced pain perception.
- Using sharp, pointed tools to remove calluses or cut toenails.
- Cutting the corners of the nails deep into the lateral nail wall, which leads to ingrown nails.
- Using hot water bottles, grain pillows or radiant heaters on the feet to compensate for feelings of coldness.
- Using over-the-counter chemical remedies for corns or calluses.
Another serious error is overlooking sock seams. Many commercial socks have thick transverse seams across the tips of the toes. Under shoe pressure, these seams act like small blades on sensitive skin. Turn such socks inside out before putting them on if seamless diabetic socks are unavailable.
Ignoring moist areas between the toes is also frequently observed. When the skin there swells and turns whitish, this is known as maceration. This softened skin is extremely vulnerable to cracks and fungal colonization.
