To withstand these stresses, the tissue requires a balanced ratio of lipid-soluble substances and moisture.
A reduction in epidermal lipids and the natural moisturising factor leads to xerosis cutis, the medical term for dry skin. If the skin lacks flexibility, it loses its biomechanical elasticity. As the mechanical load of body weight acts on the curvature of the heel with every step, the subcutaneous adipose tissue expands laterally. The overlying, rigid hyperkeratotic layer cannot plastically follow this physical expansion and cracks.
In medical terminology, these linear cutaneous fissures are referred to as rhagades or cracks. If the fissure extends only into the epidermis, it is termed a superficial rhagade. If, on the other hand, the tissue separation penetrates through the basement membrane into the dermis, free nerve endings and capillaries are damaged. This results in pinpoint or fissure-like bleeding, exuding tissue fluid, and pronounced wound pain with every mechanical compression.
Without professional intervention, the tissue at the edges of the fissure tends to undergo accelerated keratinisation. The body attempts to close the open area through increased production of corneocytes. However, this leads to a thickening of the crack margins, increasing the leverage effect under weight-bearing and driving the crack even deeper into the tissue.
Typical signs
The clinical presentation usually begins insidiously. In the early stages, reduced skin elasticity appears, accompanied by a whitish or slightly yellowish discolouration at the margin of the heel. The skin feels rough and loses its natural luster. Under pressure or lateral stretching, fine wrinkles form that no longer completely smooth out.
As the condition progresses, visible longitudinal fissure patterns develop along the stress lines of the heel. These fissure-like indentations can range from a few millimetres to several centimetres in length. If the cracks are in an advanced stage, the bases of the fissures often turn dark or reddish. This indicates dried blood as well as cellular exudates. Accompanying this is a sharp, stabbing pain, which is particularly intense when taking the first steps in the morning or after prolonged periods of rest.
The differential diagnosis requires precise differentiation from other dermatological conditions. Tinea pedis, a fungal infection of the sole of the foot, can also be accompanied by desquamation and fissure formation, but frequently presents with fine, floury-white scaling within the skin lines and occasional pruritus. Palmoplantar pustular psoriasis or atopical foot eczema also exhibit similar symptoms, but require fundamentally different pharmacological treatment approaches than a purely mechanical or dryness-induced rhagade.
Likewise, genetically determined keratodermas must be considered. In these hereditary keratinisation disorders, massive hyperkeratotic plaques develop on the hands and feet at an early age, regularly predisposing to deep fissures. A precise inspection of the skin condition by qualified specialists provides clarity regarding the primary cause.
Causes in everyday life
The causes of cracked heels are varied and frequently result from an interplay of external influences and internal factors. Among the most common triggers in everyday life is wearing inappropriate footwear. Open shoes such as mules, clogs, or sandals without heel support offer no lateral guidance to the heel fat pad. While walking, the sole slaps against the heel, leading to repetitive microtrauma and increased lateral expansion of the skin.
Likewise, walking barefoot on hard, inelastic surfaces such as tiles, concrete, or laminate plays a significant role. Lacking the shock-absorbing effect of a protective shoe sole, the focal pressure on the heel tissue increases drastically. The body responds to this physical stimulus with reactive hyperkeratosis, an increased production of horny material to protect the deeper tissue.
Internal medical factors and metabolic disorders also significantly influence the skin structure. Chronically elevated blood glucose levels in diabetes mellitus lead to reduced sweat secretion in the lower extremities via autonomic neuropathy. The skin loses its natural hydration, becomes brittle, and loses its protective barrier. Hypothyroidism and hormonal changes during menopause reduce sebum production and oestrogen levels, causing skin moisture to drop further.
Inadequate daily fluid intake, obesity with correspondingly increased static load on the heel margins, and inappropriate care practices also contribute to deterioration. Excessive use of aggressive soaps, heavily fragranced shower gels, or alcohol-containing disinfectants on the feet strips the epidermis of valuable lipids. Chemical degreasing destroys the protective acid mantle and exponentially accelerates moisture loss.
When medical evaluation is necessary
Although many heel cracks are purely mechanical in nature, there are clear warning signs that require immediate medical diagnosis and treatment. Deeply extending rhagades represent a portal of entry for pathogenic germs. Bacteria such as staphylococci or streptococci can easily penetrate into the vascularised tissue of the dermis and cause local or extensive infections.
Caution is advised if you observe redness extending beyond the immediate edge of the crack. Localised warmth of the tissue, throbbing pain throughout the heel area, purulent discharge, or swelling are also clear signs of a bacterial superinfection. In the worst-case scenario, erysipelas can develop, which is accompanied by fever and chills and must be treated systemically with antibiotics.
Special attention is required for patients with pre-existing underlying conditions such as diabetes mellitus, peripheral arterial disease, or pronounced venous drainage disorders. Impaired microcirculation or diabetic neuropathy often reduces pain perception. Cracks are noticed late and heal poorly due to impaired tissue perfusion. In these cases, an unnoticed rhagade can mark the onset of diabetic foot syndrome.
If fissures fail to heal over a period of more than three to four weeks despite consistent basic care, or if they bleed spontaneously without visible mechanical stress, a dermatological or phlebological evaluation must be conducted. This is the only way to rule out rare skin diseases or severe circulatory disorders.
What podiatric treatment can achieve

At low concentrations, urea acts as an effective moisturizer, while at higher concentrations, it serves as a keratolytic agent that gently dissolves keratinised substance.
Emulsions with a urea content of 5 to 10 percent are suitable for daily maintenance care of mildly dry skin. If pronounced hyperkeratoses and fissures are already present, medical creams or ointments containing 15 to 20 percent urea are preferred. The active ingredient draws water into the stratum corneum and softens the rigid keratin bridges between dead skin cells.
Practical implementation of the home care routine requires discipline and the right materials:
- Apply the urea-containing heel cream ideally in the evening after washing onto slightly damp skin, as active ingredients penetrate particularly well at this time.
- Use an intensifying occlusive treatment once or twice a week: apply a thick layer of a lipophilic fissure ointment and put on thin cotton socks overnight.
- For mechanical self-care, use exclusively fine-grained pumice stones on moistened skin, applying minimal pressure.
- When buying shoes, ensure closed heel counters and good outsole cushioning to reduce shock load on the heel fat pad.
- Ensure adequate hydration from within by drinking at least 1.5 to 2 litres of water or unsweetened tea daily.

A widespread misconception is the excessive use of sharp tools. Attempting to remove thick callus layers independently often leads to serious complications:
- Using corn planes, razor blades, or metal rasps creates micro-injuries in healthy tissue, which serve as a breeding ground for bacteria.
- Vigorously rubbing mechanically creates frictional heat, to which the body responds with reactive, even faster callus formation.
- Soaking the feet for hours in hot soapy baths further leaches the skin and leads to maceration, causing tissue integrity to collapse.
- Applying highly concentrated callus-removing plasters without professional supervision can cause chemical burns to the surrounding, thinner skin areas.
- Becoming negligent with the daily moisturizing routine as soon as initial pain subsides quickly leads to a relapse into the old condition.
Aggressive mechanical removal of skin layers in the area of existing rhagades poses particular dangers. If the edge of the crack is torn uncontrollably, the fissure inevitably propagates into deeper, living skin layers. This leads to painful bleeding and drastically increases the risk of infection.
