What lies behind it
In the vast majority of cases, an inadequate supply of oxygen-rich blood to the legs is due to peripheral arterial disease, abbreviated as PAD. Progressive atherosclerosis leads to narrowing or even complete occlusion of the arterial vessels. Over years, the inner layer of the arterial wall, known as the tunica intima, deposits lipids, calcium and connective tissue. These plaques steadily reduce the internal diameter of the vessel. Blood flow stagnates, leaving downstream tissue areas insufficiently supplied with oxygen and essential nutrients.
Physicians traditionally divide the course of this disease into four clinical stages according to the Fontaine classification. In stage I, a demonstrable vascular change is already present, but patients remain free of symptoms. Stage II is characterised by typical exercise-induced pain, known as intermittent claudication. While in stage IIa the pain-free walking distance is still more than 200 metres, in stage IIb it falls below the 200-metre mark. In stage III, pain occurs even at rest, particularly when the legs are elevated flat during night-time rest. Stage IV is characterised by trophic tissue damage, ranging from poorly healing ulcerations to tissue conditions such as gangrene.
In addition to macroangiopathic narrowing of the major arteries such as the femoral artery or tibial artery, microcirculation can also be damaged. In individuals with concomitant diabetes mellitus, medial calcification frequently occurs, which is a stiffening of the middle vascular wall layer. This combination severely impairs tissue supply. Cells enter a state of chronic hypoxia, causing cellular metabolism to stall and leaving the local immune system barely capable of defense.
Typical signs
The symptoms of impaired arterial circulation in the foot are diverse and require careful inspection. In addition to exercise-dependent muscle pain in the calf, thigh or buttocks, the skin of the foot provides clear indications. Marked skin paleness when the legs are elevated is characteristic. When the legs hang down, a cyanotic, reddish-blue discolouration often appears as blood pools in the dilated capillaries.
Skin temperature provides further valuable insights. A foot that is permanently cool, either unilaterally or bilaterally, indicates reduced perfusion. The capillary refill time serves as one method of assessment. The toe pad is pressed briefly until it pales. If more than two to three seconds elapse after releasing the pressure before the pink colour returns, this indicates delayed microvascular refilling.
The nail unit also undergoes severe changes under oxygen deficiency. Toenails grow significantly slower, become brittle, turn yellowish-brown or undergo hypertrophic thickening. Hair on the dorsum of the toes and foot is often completely lost. The skin appears thin, shiny like parchment, and tends towards extreme dryness because sweat and sebaceous glands also receive insufficient blood flow.
A clear differentiation from venous insufficiency is essential. Venous circulatory disorders usually lead to swollen, warm legs with hyperpigmentation and a feeling of heaviness, which improve when the legs are elevated. Arterial deficits, on the other hand, worsen when the legs are elevated, as the hydrostatic pressure required to overcome vascular bottlenecks is lacking.
Everyday causes
The causes of arterial vessel occlusion are closely linked to individual lifestyle habits and existing systemic conditions. The most significant modifiable risk factor is tobacco use. Components of cigarette smoke directly damage the vascular endothelium, promote inflammatory responses and significantly accelerate atherosclerosis.
Diabetes mellitus also plays a central role. Permanently elevated blood sugar levels damage the inner vascular wall via advanced glycation end products. Combined with high blood pressure and lipid metabolism disorders, a highly harmful environment for the arterial system develops. Physical inactivity exacerbates these effects, as the lack of muscle stimulation prevents the formation of collateral vessels.
In the immediate foot area, everyday factors often act as catalysts for complications. Tight or overly short footwear exerts continuous pressure on hypoxic skin. Because the tissue is extremely pressure-sensitive due to inadequate perfusion, blisters or microtrauma quickly develop. If these areas are further constricted by improper socks with cutting elastic bands, local perfusion deficits increase drastically.
Footcare errors also play a significant role. Improper trimming of toenails using sharp instruments easily causes minor cuts near the nail fold. While such injuries heal without scarring within a few days in a healthy individual, they can serve as the starting point for a chronic, non-healing wound in patients with PAD.
When medical evaluation is necessary
Impaired arterial circulation must never be taken lightly, as it drastically increases the risk of tissue loss and cardiovascular events such as myocardial infarction or stroke. As soon as warning signs appear, immediate specialist evaluation by angiologists, vascular surgeons or diabetologists is required.
Rest pain is an absolute warning signal. If the foot hurts particularly at night or when the body is in a horizontal position, and relief is obtained only by dangling the leg out of bed, an advanced Fontaine stage III is already present. At this stage, tissue necrosis is imminent.
Skin defects, small cracks or blisters that show no signs of healing after two weeks are equally urgent. If black, necrotic skin areas appear on the tips of the toes or heels, this is a medical emergency. In the event of a sudden arterial occlusion, Pratt's classic six Ps occur: pain, pallor, pulselessness, paresthesia, paralysis and prostration (shock). In this situation, emergency services must be contacted immediately.
Medically, the diagnosis is usually confirmed primarily by measuring the ankle-brachial index, abbreviated as ABI. An ABI value below 0.9 confirms the presence of PAD, while values below 0.5 indicate critical limb ischaemia. Supplementary ultrasound examinations and angiographies precisely locate the stenosis.
What podiatric treatment can achieve
In patients with peripheral circulatory disorders, the podiatry practice performs a central protective and monitoring function. As a specialised medical profession, podiatry is trained to treat vulnerable feet atraumatically and to prevent skin damage at all costs. Prior to any treatment, a thorough clinical assessment is conducted, which includes palpating the pedal pulses at the dorsalis pedis artery and posterior tibial artery.

The instrumentation in the practice differs fundamentally from cosmetic tools. Only sterile single-use scalpel blades and rotating diamond burrs with wet spray technique are used. Thickened toenails are thinned down in a controlled manner to a physiological level in order to minimise pressure on the nail bed. In addition to professional care, patients can contribute significantly to protecting tissue and slowing disease progression through their daily habits. Independent self-monitoring is paramount.
Impaired arterial circulation requires discipline and attentiveness in daily life. The following guidelines support the maintenance of foot health:
- Perform a daily visual check of your feet, using a hand mirror for the soles if necessary, or ask family members for assistance.
- Wash your feet briefly every day with lukewarm water below 37 degrees Celsius and dry between the toes thoroughly by gently patting dry.
- Apply a moisturising, urea-containing lotion to your feet daily, omitting the interdigital spaces between the toes to avoid maceration.
- After consulting your physician, complete structured walking exercise to systematically stimulate the formation of collateral circulation.
- Wear soft, seamless shoes with adequate room for the toes and refrain entirely from walking barefoot to avoid injury.
Controlled walking exercise is one of the most effective means of extending pain-free walking distance in stages I and II of PAD. Walking close to the pain threshold provides the stimulus for creating new capillaries. However, if rest pain or open wounds are present, any exercise is strictly prohibited to avoid increasing tissue oxygen demand further.
Common errors
Clinical practice repeatedly demonstrates that well-intentioned care measures can severely damage tissue. A classic mistake is using hot foot baths. Because arterial circulatory disorders are often accompanied by reduced temperature sensitivity, excessively hot water temperatures are not perceived in time. This easily leads to second- or third-degree burns, which hardly heal due to ischaemia.
Another significant risk is the use of over-the-counter corn plasters. These preparations usually contain salicylic acid, which chemically dissolves horny tissue. While this is generally harmless on healthy skin, the acid often burns through to deep skin layers in tissue with poor blood flow, creating deep ulcerations.
Applying tight bandages or compression stockings without prior angiological evaluation is equally dangerous. If a compression stocking is worn in undetected severe PAD, it completely cuts off the remaining arterial blood flow. Extensive tissue necrosis occurs within a short period. Finally, using sharp household tools such as razor blades, callus planes or foot rasps regularly results in injuries with catastrophic consequences.
Current evidence and clinical context
Medical evidence underscores the necessity of an interdisciplinary treatment approach for peripheral circulatory disorders. The German S3 guideline on peripheral arterial disease emphasizes the paramount importance of risk factor control and regular foot inspection [1]. Epidemiological data show that approximately 20 percent of the population over 65 years of age in Germany is affected by PAD, with a high unreported figure due to asymptomatic stage I progression [2].
Clinical studies clearly demonstrate that structured podiatric care for high-risk patients can significantly reduce amputation rates. Through early identification of pressure points and atraumatic treatment, tissue defects are prevented before developing into chronic wounds [3]. In addition, the positive effect of supervised walking exercise on microcirculation and quality of life is confirmed in numerous meta-analyses [4]. The pitfall of silent ischaemia in concomitant diabetic neuropathy
If peripheral arterial disease occurs together with diabetic polyneuropathy, the risk profile changes dramatically. Medical professionals refer to this condition as a neuroischaemic foot. The damaged nerve fibres transmit pain signals to the brain only with delay or not at all. Consequently, the classic cardinal symptom of PAD, cramp-like exertional pain when walking, is completely absent.
In this scenario, vascular narrowings often progress unnoticed for years. The absence of a warning signal leads affected individuals to continue full weight-bearing on their tissue despite massive oxygen debt. A striking example from clinical practice illustrates the risk: a 68-year-old patient from the Allgäu region completed extensive daily walks without experiencing any calf pain. However, the popliteal artery was already completely occluded at that time. A tiny pebble in the shoe exerted pressure on the heel for hours. Due to numb skin nerves, the micro-injury went unnoticed and developed into a deep ulcer within a few weeks.
The lack of pain perception lulls patients into a false sense of security. Blisters, pressure points or small cuts are often discovered only when the tissue has already turned dark or signs of infection, such as redness at the margin of the foot, become visible. Regular sensitivity testing using a monofilament and tuning fork is therefore mandatory for all diabetic patients.
Offloading and orthopaedic footwear provision
Reducing mechanical pressure peaks is a fundamental requirement in existing circulatory disorders to prevent tissue breakdown at exposed foot areas. The sole of the foot absorbs multiples of body weight with every step. In a narrowed arterial pathway, poorly supplied tissue cannot withstand these shear forces.
Precise offloading requires exact collaboration between podiatry and orthopaedic footwear technology. Using digital pedobarography, pressure distribution beneath the sole of the foot is measured dynamically during walking. Areas with critical pressure peaks exceeding 30 kilopascals can be identified precisely in this way. Based on these findings, individual diabetes-adapted footbeds are fabricated. Multi-layered composite materials are used for this purpose. Soft foams such as ethylene-vinyl acetate with a low Shore hardness of 15 to 20 Shore A cushion impacts in the forefoot area, while firmer layers support midfoot mechanics.
In addition, ready-made or custom-manufactured specialized shoes featuring a rigid sole stiffener and a rocker bottom are used. This construction reduces bending movement in the metatarsophalangeal joints during toe-off, drastically decreasing pressure on the tip of the foot. For acute lesions, temporary offloading shoes or custom orthoses are utilised. They completely suspend pressure over the affected area so that blood circulation can utilise the remaining regenerative potential for wound healing without mechanical disruption.
Phase-appropriate wound management in ischaemic defects
If an open wound develops on an underperfused foot despite all precautions, highly specific wound management is required. A fundamental treatment error consists in managing ischaemic wounds analogously to well-perfused lesions. In PAD, arterial status decisively determines the choice of wound dressing and surgical approach.
In dry, necrotic tissue changes on the toes without signs of infection, the principle of dry preservation applies. The necrotic cap protects underlying tissue from germ ingress. Surgical debridement to remove dead tissue should be performed only after successful vascular intervention, such as percutaneous transluminal angioplasty or bypass surgery, has restored circulation. If dry necrosis is kept moist or incised prematurely without prior vascular reconstruction, immediate bacterial entry and conversion into wet gangrene are threatened.
As soon as a chronic, sloughy wound requires cleansing in the presence of adequate hemodynamics, hydroactive wound dressings are applied. Polyurethane foams and alginates absorb wound exudate, maintain a moist wound environment and promote granulation tissue formation. In cases of bacterial colonization, silver-containing compresses or medical honey are used to combat infection. How do I recognise a circulatory disorder in my feet?
Typical signs include cool skin, pale or reddish-blue discolouration, and slow-growing, brittle nail tissue. If small pressure sores or wounds fail to heal over several weeks, this is an urgent warning signal. Detailed investigation by a vascular specialist is essential in these cases.
Can I take a foot bath if I have PAD?
Foot baths should only be used with great caution and after consultation if circulatory disorders are present. The temperature must be checked with a bath thermometer and must not exceed 37 degrees Celsius. As thermal sensitivity is often impaired, there is otherwise a risk of severe scalding. Furthermore, the bath should not last longer than three to five minutes to avoid macerating the skin.
Why are corn plasters dangerous in cases of poor circulation?
Corn plasters contain highly concentrated acids, such as salicylic acid, which chemically dissolve the hyperkeratotic tissue. In poorly perfused skin, the body cannot defend against or repair this chemical attack. The acid often penetrates deep tissue layers and creates open ulcerations with an extremely high risk of infection. Calluses and corns in patients with PAD should be removed exclusively non-invasively and professionally in a podiatry practice.
How does podiatric care differ from cosmetic foot care?
Podiatrists complete several years of state-regulated training and are medically qualified to treat high-risk patients with vascular or nerve damage. In podiatry, only sterile instruments and specially cooled rotary burrs are used to prevent any micro-injury. In addition, the podiatrist performs a thorough medical history and clinical assessment to identify risks early. Cosmetic foot care, by contrast, is limited to aesthetic treatments on healthy feet.
Does health insurance cover the cost of podiatric treatment?
In cases of advanced peripheral arterial disease in stage III or IV with an existing wound risk, the attending physician can issue a prescription for remedies (Heilmittelverordnung). In this case, statutory and private health insurance funds cover the costs for podologische Komplexbehandlung (comprehensive podiatric treatment) according to applicable guidelines.
