The phenomenon of burning feet, frequently referred to in medical terminology as burning feet syndrome, is based on complex dysfunction or damage to the peripheral nervous system as well as disorders of microvascular blood flow. To understand the development of these symptoms, it is worth looking at the neuroanatomy of the lower leg and foot. Sensation in the skin on the soles of the feet is mediated by a dense network of nerve fibres. The density of unmyelinated C fibres and thinly myelinated A-delta fibres is particularly high. These thin nerve fibres are responsible for transmitting pain and temperature sensations. When these specific fibres are damaged or chronically irritated, specialists refer to it as small-fibre neuropathy. The affected nerves send faulty, uncontrolled impulses to the central nervous system. The brain interprets these erroneous signals as an intense heat signal or a burning pain, even when the skin surface is physically cool.

In addition to damage to small nerve fibres, vascular changes play a central role. Peripheral nerves are supplied by tiny blood vessels, the vasa nervorum. If tissue microcirculation is impaired, the nerve fibres suffer from a chronic lack of oxygen and nutrients. This is frequently the case in peripheral arterial disease as well as diabetic angiopathies. Erythromelalgia represents a special form, a rare vascular disease in which episodic dilation of the small arterioles occurs. This leads to severe overheating, redness, and burning pain in the extremities.

Another anatomical mechanism involves mechanical nerve compression. The tibial nerve runs along the inside of the ankle joint through a narrow bony and fibrous canal, the tarsal tunnel. If narrowing occurs in this area due to swelling, malalignments, or scar tissue, tarsal tunnel syndrome develops. Affected individuals primarily report a deep-seated, glowing heat sensation on the soles of the feet that intensifies in phases. Frequently, these abnormal sensations do not occur in isolation, but are accompanied by further paraesthesias. These include a tingling numbness, a feeling like walking on cotton wool or fine sand, as well as sudden, electrifying stabs of pain.

A characteristic feature of many neuropathic conditions is so-called allodynia. Here, stimuli that are normally painless trigger intense discomfort. Mere contact of the duvet with the soles of the feet or wearing light socks is perceived by those affected as a painful burning sensation. Hyperalgesia also frequently occurs, in which mildly painful stimuli are felt with excessive intensity.

The temporal dynamics are of high importance for diagnostic classification. Many patients report a marked worsening of symptoms during the evening and night hours. As the body comes to rest, distracting stimuli of the day fall away. At the same time, the body's cortisol level decreases according to the circadian rhythm, which can amplify pain perception.

Differentiation from related clinical conditions requires precise examination. While restless legs syndrome is dominated by an irresistible urge to move the legs, which is temporarily relieved by movement, pure burning in small-fibre neuropathy often persists even during movement. Raynaud's phenomenon, on the other hand, exhibits a typical triphasic colour change of the skin from white to blue to red, accompanied by feelings of coldness.

A case example from podiatric practice illustrates this progression. A 62-year-old patient presented with nocturnal burning pain in both forefeet that had persisted for eight months. The skin appeared visually unremarkable, but the podiatric sensibility test using a monofilament revealed distinct deficits in perception at metatarsal heads I and V. Subsequent medical specialist evaluation confirmed previously undetected diabetic polyneuropathy.

Everyday causes

The causes of burning feet are manifold, ranging from systemic underlying conditions to mechanical strain in everyday life. By far the most common trigger for peripheral nerve damage is diabetes mellitus. Chronically elevated blood glucose levels lead via complex biochemical cascades to the formation of so-called advanced glycation end products. These protein-sugar compounds deposit in the vessel walls of the vasa nervorum and directly damage nerve structures.

Metabolic and toxic influences play an equally significant role. A deficiency in vitamin B12 (cobalamin) leads to demyelination of nerve fibres. Such a deficiency occurs not only with a vegan diet, but frequently with long-term use of proton pump inhibitors for stomach acid reduction or the diabetes medication metformin. Deficiencies in vitamin B1, B6, and folic acid can also trigger neuropathic symptoms. Conversely, excessive high-dose intake of vitamin B6 can also have a toxic effect on peripheral nerves.

Further systemic causes include chronic renal insufficiency with accumulation of uraemic toxins in the tissue, hypothyroidism, and regular alcohol consumption with a direct toxic effect on Schwann cells. Autoimmune diseases such as Sjögren's syndrome or infectious diseases such as Lyme disease and shingles can also trigger neuropathies.

In addition to systemic factors, mechanical triggers play an important role. Footwear that is too tight or ill-fitting exerts continuous pressure on interdigital nerve pathways. High-heeled shoes shift body weight onto the forefoot, leading to overloading of the metatarsal heads and promoting nerve irritation. Foot deformities such as splayfoot, flatfoot, or hallux valgus also alter structural mechanics in such a way that nerve trunks are subjected to tensile stress.

Care errors in daily routine often unintentionally worsen symptoms. Applying highly occlusive ointments during nocturnal burning impairs the dissipation of skin heat and leads to heat buildup. Likewise, using hard pumice stones or blades to remove calluses when pain perception is reduced often leads to microtrauma, which can become infected and trigger secondary nerve irritation.

When medical evaluation is necessary

Burning feet are not merely a cosmetic or temporary discomfort. They represent a warning sign from the nervous or vascular system. Prompt medical diagnosis is imperative to prevent progressive damage.

Urgent action is required particularly when the following warning signs appear:

  • Sudden onset of burning or rapid spread of symptoms within a few days
  • Additional motor deficits, such as foot drop or frequent tripping
  • Unilateral occurrence of symptoms, pointing to acute nerve compression or a vascular event
  • Visible skin changes, discolouration of the toes, open wounds, or blistering
  • Absence of foot pulses or marked paleness of the limb when elevated
  • Fever or general signs of illness combined with foot symptoms

The neurologist uses procedures such as electromyography, nerve conduction velocity measurement, and quantitative sensory testing to determine the precise extent of nerve damage. What podiatric treatment can achieve

In podiatric practice, a specialised, non-invasive assessment and symptomatic relief of the foot takes place. Podiatric treatment effectively complements medical specialist therapy to prevent secondary damage and improve the quality of life for those affected.

The initial appointment begins with a structured podiatric history and a thorough examination of the skin and vascular status. Standardised test procedures are used to evaluate sensory nerve function. Vibration perception is tested at bony prominences such as the first metatarsophalangeal joint using a Rydel-Seiffer tuning fork (128 Hz). After washing, the feet, especially between the toes, must be dried thoroughly with a soft towel to prevent fungal infections.

Subsequent skin care requires suitable care products. Medical foot creams or foam creams with a urea content of 5 to 10 per cent are recommended. Urea binds moisture in the stratum corneum and keeps the skin supple. The spaces between the toes should be avoided when applying cream to prevent skin maceration.

The following criteria should be considered when purchasing shoes:

  • Sufficient room in the forefoot area so that the toes are not restricted
  • Flexible, shock-absorbing soles to reduce pressure peaks
  • Avoidance of internal seams that could lead to chafing
  • Breathable upper materials such as soft leather or mesh fabrics
  • Fitting shoes to the foot in the afternoon, as feet tend to swell slightly during the day

The choice of socks also has a major influence. Socks should be made of natural, moisture-regulating fibres such as merino wool or high-quality cotton. They must not have tight elastic bands that restrict venous return, and they should be seamless.

Common errors

Daily practice repeatedly reveals recurring behaviours by patients that unintentionally worsen symptoms or result in severe tissue damage.

An extremely dangerous error is applying direct heat sources to burning feet. Because patients often misinterpret the burning sensation as a feeling of cold or seek relaxation, many resort to hot water bottles, heating pads, or hot foot baths. When sensory impairment is present, heat is not adequately perceived. Severe second- and third-degree burns regularly occur, which can lead to chronic ulcers due to poor wound healing.

Equally risky is self-removal of calluses or corns using blades, rasps, or over-the-counter corn plasters. These plasters contain highly concentrated salicylic acid, which dissolves keratinised tissue. In thin or neuropathic skin, the acid penetrates unimpeded into deeper tissue layers, causing chemical burns and profound tissue necrosis.

Ignoring early symptoms represents another error. Nocturnal tingling or mild burning is often dismissed as an insignificant sign of ageing or fatigue. The longer nerve irritation or a metabolic disorder remains untreated, the higher the risk of irreversible damage to nerve fibres.

Walking barefoot on uneven or unfamiliar ground also carries risks. With reduced protective sensation, small foreign bodies such as stones, glass splinters, or wood splinters go unnoticed. They remain in the tissue and lead to unnoticed, progressive infections.

Current evidence and medical context

Scientific research into peripheral neuropathies and microvascular disorders has made significant progress in recent years. Research findings demonstrate that small-fibre neuropathy was long underestimated as a diagnosis, as standard electrophysiological examinations such as nerve conduction velocity primarily assess thickly myelinated A-alpha and A-beta fibres [1]. Targeted diagnostics of thin fibres require specialised procedures such as the measurement of intraepidermal nerve fibre density.

Clinical studies show that early glycaemic control in diabetic patients significantly reduces the risk of onset and progression of peripheral neuropathy [2]. Movement brings immediate relief in restless legs syndrome, whereas this is not the case with purely neuropathic burning feet.

Are cold foot baths advisable for burning feet?

Ice-cold foot baths are not recommended for burning feet.
