What lies behind the condition

Psoriasis vulgaris is a chronic inflammatory autoimmune disease characterised by disrupted epidermal renewal. While healthy skin cells renew within approximately 28 days, this process is shortened to a few days during active psoriasis. Keratinocytes divide uncontrollably and migrate rapidly to the skin surface, where they undergo premature cornification. This leads to characteristic plaque formation featuring silvery-white scales on an inflamed, reddened base.

On the feet, the condition frequently appears as palmoplantar psoriasis, which specifically affects the palms of the hands and soles of the feet. Due to continuous mechanical stress during walking and standing, the clinical appearance on the sole of the foot often differs from that on other areas of the body. Instead of loose layers of scales, thicker and sometimes cracked hyperkeratotic plaques develop, referred to medically as a hyperkeratotic-rhagadiform pattern.

When the disease involves the nail apparatus, it is termed psoriasis unguium or nail psoriasis. The changes depend on which anatomical structure of the nail is affected by the inflammatory process. If inflammation originates in the nail matrix, where nail growth is generated, disturbances in the structure of the nail plate develop. Conversely, if the process affects the nail bed beneath the plate, discolouration and tissue accumulation manifest under the nail.

The nail matrix forms the keratinous substance of the nail plate. If the cellular clusters there become inflamed in phases, small areas of dyskeratosis develop, which fall out as the nail grows, remaining visible as pinpoint depressions. If the inflammatory focus is located in the distal nail bed, serum and cellular extravasates accumulate under the plate. These subtle processes illustrate that nail psoriasis is by no means merely a keratinisation disorder, but rather the result of complex immunological processes within the tissue.

Typical signs

The visual appearance of psoriasis on the feet and nails is diverse and varies depending on the activity of the flare-up. On the skin, sharply demarcated, reddish areas predominate, often covered with firmly adherent scales. In the region of the heels and the balls of the feet, deep skin fissures, known as rhagades, readily form. These are extremely painful and serve as entry portals for bacterial pathogens.

On the nails, clear key symptoms characteristic of nail psoriasis manifest:

  • Pitting: Pinhead-sized, pinpoint depressions in the nail plate caused by shedding parakeratotic cell complexes.
  • Oil drop sign (salmon patch): Yellowish-brown to reddish discolouration beneath the nail plate caused by inflammatory exudates in the nail bed.
  • Subungual hyperkeratosis: Crumbly, crusty accumulations of keratin beneath the distal edge of the nail, which can elevate the nail plate.
  • Onycholysis: Partial detachment of the nail plate from the nail bed, usually originating at the free edge, often bordered by a reddish margin.
  • Severe nail dystrophy (crumbly nails): Complete destruction of the nail structure in very severe cases, where the nail plate crumbles.

Differential diagnosis between nail psoriasis and onychomycosis (fungal nail infection) presents a challenge even for experienced practitioners. Both conditions lead to thickening, yellow discolouration, and brittle substance within the nail area. A decisive difference often lies in the pattern of symmetry. While psoriasis on the nails usually affects multiple nails symmetrically on both feet, a fungal nail infection typically begins on a single nail and slowly spreads to adjacent toes. Furthermore, fungal nail infections lack the typical oil drop sign with its circumscribed marginal border.

Nevertheless, the two conditions are not mutually exclusive. Tissue altered by psoriasis represents a compromised barrier that can easily be colonised by dermatophytes or yeasts. This is referred to as a secondary infection. To establish certainty, a microscopic examination alongside a fungal culture or PCR analysis carried out by a dermatology practice is essential.

Causes in everyday life

The occurrence of flare-ups on the feet is significantly influenced by external factors. A central role is played by the Koebner phenomenon, also known as the isomorphic response. In individuals with a corresponding genetic predisposition, mechanical, thermal, or chemical stimuli on previously healthy skin can trigger typical psoriasis plaques. On the feet, this stimulus is continuously generated by friction and pressure.

Ill-fitting footwear is one of the most common everyday triggers. Shoes that are too tight compress the toes, generating increased pressure on the nail matrix and nail fold. Shoes that are too loose, in which the foot slides forward with every step, also lead to repetitive microtrauma to the tips of the toes. These constant minimal impacts are sufficient to initiate the Koebner effect and fuel a new flare-up.

Foot deformities also negatively affect the course of the condition. Hallux valgus, fallen arches or splay feet, and hammer toes alter pressure distribution under the sole of the foot. At points of increased biomechanical stress, the skin responds with excessive callus formation. When combined with a psoriasis-typical keratinisation disorder, particularly thick, inelastic layers form, which crack deeply during the roll-off motion of the foot.

Improper home foot care habits often inadvertently exacerbate the problem. Aggressive cleaning of the nail fold or the underside of the free edge with sharp metal instruments damages the sensitive hyponychium. These minor tissue injuries directly worsen nail detachment or trigger new psoriasis foci beneath the nail. The use of drying soaps or highly concentrated foot baths also strips valuable lipids from the already compromised skin barrier.

When medical evaluation is necessary

Psoriasis pedis is a systemic condition that requires medical supervision. A purely cosmetic or care-focused approach is insufficient, as inflammatory processes take place throughout the entire body. Consulting a dermatology practice is essential to secure an accurate diagnosis and, if necessary, initiate prescription systemic or topical pharmacological therapy.

Particular attention must be paid to the onset of joint discomfort. Approximately 20 to 30 percent of people with psoriasis develop psoriatic arthritis over the course of their lives. If toe joints become painful, swollen, or feel stiff in the morning, this is a clear warning signal. A so-called "sausage digit", known medically as dactylitis, in which the entire toe is severely swollen and erythematous, requires an immediate referral to rheumatology. Early treatment can prevent permanent joint damage.

Likewise, signs of a bacterial wound infection require immediate medical action. Heel rhagades or severely detached nails provide an ideal entry portal for bacteria such as staphylococci. Signs of infection include rapidly increasing redness, local heat, throbbing pain, pus formation, or even fever. In such cases, prompt antibiotic treatment is necessary.

What podiatric treatment can achieve

Professional foot care within a podiatry practice represents an essential complement to dermatological therapy. The primary goal is to relieve mechanical stress on damaged skin and nails, alleviate pain, and minimise the risk of secondary complications. Each treatment begins with a meticulous clinical assessment. The condition of the skin, the thickness of the nail plates, any nail detachment, and the presence of rhagades are precisely documented. For thickened nail plates with subungual hyperkeratosis, the podiatrist uses specialised rotary instruments. Diamond burs or fine-grained ceramic cutters are used, operating at low speeds with a cooling spray mist. Continuous cooling prevents any heat generation, as thermal stimuli could trigger a flare-up via the Koebner phenomenon.

The crumbly, excess keratin material under the nail plate is gently and painlessly removed without further irritating the sensitive nail bed. In cases of severely detached nail sections, the unstable material is shortened to the point where shearing and tensile forces no longer act on the remaining nail bed. This reduces the risk of the nail catching on socks and tearing painfully.

On the skin, gentle pressure relief is the primary focus. Excessive hyperkeratosis on the heels or balls of the feet is professionally smoothed using scalpels and files without injuring the deeper epidermis. For painful rhagades, specialised dressings or protective padding (tamponades) can be applied. Treatment intervals depend on the individual clinical presentation and flare-up activity. During active flare-ups, intervals of three to four weeks have proven effective to interrupt the formation of severe hyperkeratosis at an early stage. During symptom-free intervals, appointments can be extended to five to eight weeks.

What you can do yourself

Home care forms the foundation for long-term stabilization of the skin barrier between practice visits. The goal is to increase skin moisture content, soothe inflammation, and gently loosen layers of scales.

When selecting skin care products, preference should be given to formulations specifically developed for compromised skin barriers. Effective ingredients include:

  • Urea: At a concentration of 5 to 10 percent, urea intensively binds moisture within the stratum corneum. Higher concentrations above 15 percent should only be used during an acute flare-up after consulting a professional, as they can sting on open areas.
  • Glycerine and ceramides: These lipids repair the natural lipid barrier of the skin and prevent trans-epidermal water loss.
  • Micro-silver: Has a mild antimicrobial effect and calms irritated skin areas prone to inflammation.
  • Evening primrose oil or almond oil: Provide essential fatty acids to relieve itching.

Daily foot care should become a consistent routine. Ideally, apply cream gently after showering or washing while the skin is still slightly damp. Largely avoid applying cream between the toes to prevent a moist environment that promotes fungal infections. For deep heel rhagades, targeted application of specialised skin ointments or medical petrolatum before bed is recommended, followed by wearing thin cotton socks overnight.

Lukewarm water with lipid-replenishing, soap-free washing lotions is sufficient for cleaning the feet. Avoid long, hot foot baths. A foot bath should last no longer than five to ten minutes and should not exceed a water temperature of 36 degrees Celsius. Sea salt additives or botanical oils can be added to the bathwater. After bathing, gently patting the skin dry with a soft towel is essential, rubbing must be strictly avoided.

For nail care, the rule applies that nails should never be cut too short and the corners should not be rounded off deeply at the sides, in order to prevent ingrown nails. Prefer fine glass or emery board files over nail clippers or scissors, as these cause fewer micro-cracks in the nail tissue. Never clean the area beneath the nail using sharp metal nail cleaners.

Common mistakes

In daily clinical practice, we regularly encounter mistakes made with the best intentions that drastically worsen psoriasis symptoms. The most common error is attempting to self-remove calluses and scales using abrasive tools such as pumice stones, callus rasps, or razor blades. Strong mechanical pressure and uncontrolled ablation cause micro-injuries to the skin. Via the Koebner phenomenon, these immediately trigger accelerated keratinocyte division. The result is an even thicker hyperkeratotic plaque a few days later.

Another critical issue is the unauthorized use of highly concentrated callus remover solutions or salicylic acid plasters from the chemist. These preparations have a strong caustic effect, softening not only the callus but also the surrounding healthy tissue. Deep chemical burns and painful inflammation ensue, massively increasing the risk of bacterial superinfections.

Equally damaging is manually scraping out crumbly material beneath the nail plate with nail files, toothpicks, or paperclips. This mechanical manipulation further detaches the nail from the nail bed, creating cavities where pathogens proliferate. Tearing off loose skin scales by hand also frequently tears deeper tissue layers.

Finally, applying over-the-counter anti-fungal nail lacquers to thickened nails across the board without prior medical evaluation often leads to wasted time and tissue irritation. If no fungal infection is present, the chemical lacquer layer places unnecessary strain on the psoriatic nail bed.

An anonymised case report illustrates this. A 48-year-old female patient from the district of Unterallgäu spent months trying to treat her discoloured and thickened toenails with over-the-counter antimycotics and a metal file. Constant filing and scraping beneath the nail plate led to the complete detachment of two nail plates and severe pain. Only after specialist diagnosis of psoriasis unguium and a switch to gentle podiatric nail reduction along with a corticosteroid nail tincture did the nail bed permanently settle.

Scientific evidence and context

Medical evidence regarding the treatment of psoriasis pedis and psoriasis unguium has strengthened significantly in recent years. The current S3 guideline on the management of psoriasis vulgaris emphasises that nail psoriasis is not merely a cosmetic issue, but a strong predictor for the subsequent development of psoriatic arthritis [1]. Early diagnosis and consistent treatment of nail changes therefore also hold preventive relevance in rheumatology.

Investigations evaluating the severity of nail changes utilise standardized instruments such as the Nail Psoriasis Severity Index (NAPSI). Studies show that isolated topical treatment of nails using vitamin D3 analogues or high-potency glucocorticoids, when applied regularly over a period of at least six months, leads to significant improvements in NAPSI scores [2]. However, the slow growth rate of toenails (only about 1 to 1.5 millimetres per month) demands a high degree of patience from affected individuals.

The importance of professional podiatric supportive care is increasingly highlighted in the medical literature. Clinical studies demonstrate that expert, atraumatic thinning of psoriatic nail plates significantly improves the penetration of topical therapeutics into the nail matrix and nail bed [3]. By reducing hyperkeratotic deposits, active substances can reach deeper tissue layers, increasing the efficacy of prescribed local therapy.

Furthermore, recent studies on biologic therapies demonstrate that modern systemic therapeutics achieve high response rates even in hard-to-treat cases of palmoplantar psoriasis [4]. Frequently asked questions

Can nail psoriasis be cured completely?

A complete cure in terms of permanently removing the genetic predisposition is not currently possible, as psoriasis is a chronic autoimmune condition. However, through a combination of modern dermatological therapy, appropriate podiatric treatment, and consistent care, symptoms can often be fully suppressed. The nails can grow back healthy, provided the inflammation in the matrix is controlled.

How do pitted nails differ from Beau lines or transverse ridges?

Pitted nails are tiny, pinhead-sized depressions distributed randomly across the nail plate, caused by localised keratinisation disorders in the nail matrix. Transverse ridges or Beau lines, by contrast, run horizontally across the entire width of the nail and indicate a temporary, systemic interruption of nail growth, such as after a high fever or severe infection. Can you go to a swimming pool with psoriasis on your feet?

In principle, there is no reason not to visit a swimming pool, provided there are no open, inflamed rhagades or bacterial superinfections. However, chlorinated water can dry out the skin further, which is why showering thoroughly and applying moisturising lipids afterwards is essential. Wearing pool shoes also protects against mechanical pressure and infections.

How long does it take for toenail changes to heal?

Because toenails grow very slowly, regeneration requires considerable patience. A toenail grows on average only about 1 to 1.5 millimetres per month. It therefore often takes nine to twelve months for damage originating in the matrix to grow out completely and for the nail plate to look healthy again.

Does cold therapy help with psoriasis on the feet?
